2型糖尿病患者合并抑郁状态的相关因素调查分析

Investigation and analysis of related factors of depression in type 2 diabetic patients

    目的研究2型糖尿病(T2DM)患者抑郁情绪患病情况以及T2DM患者合并抑郁状态的相关影响因素。
    方法采用随机抽样的方法调查复旦大学附属中山医院青浦分院内分泌科门诊和住院部T2DM患者231例,采用自制的一般资料调查表、抑郁自评量表(SDS)和焦虑自评量表(SAS)进行调查。使用SPSS 23.0软件进行统计分析。
    结果T2DM患者抑郁症状患病率为29.9%,男女间差异无统计学意义(χ2=1.763,P=0.195)。将单因素有意义的三个因素:婚姻状况、周围人对您患病情况的关注、合并焦虑纳入回归模型,最终得到婚姻状况、合并焦虑对患者的抑郁状况有影响,其中已婚者相比于未婚、离婚或丧偶者而言发生抑郁症状的概率低,已婚患者发生抑郁的概率是未婚、离婚或丧偶者发生抑郁症状概率的0.184倍。合并焦虑者相比于没有合并焦虑者而言发生抑郁症状的概率高,合并焦虑者发生抑郁症状的概率是没有合并焦虑者发生抑郁症状概率的9.914倍。
    结论2型糖尿病患者的婚姻状况以及是否合并焦虑是T2DM患者合并抑郁状态的影响因素,且未婚、离婚或丧偶的T2DM患者易发生抑郁症状,合并焦虑的T2DM患者也容易发生抑郁情绪,是心理干预的重点人群。

     

    Abstract:
    ObjectiveTo study the prevalence of depression in type 2 diabetes mellitus (T2DM) patients and the related factors of depression in T2DM patients.
    MethodsA total of 231 T2DM patients in the Department of Endocrinology, Qingpu Branch of Zhongshan Hospital, Fudan University were investigated by random sampling.Self-rating Depression Scale (SDS) and Self-rating Anxiety Scale (SAS) were used for the investigation.The data were analyzed by SPSS 23.0 software.
    ResultsThe prevalence of depressive symptoms in T2DM patients was 29.9%, and there was no significant difference between males and females (χ2=1.763, P=0.195).Marriage status, concern of people around you, and combined anxiety were included in the regression model.Marriage status and combined anxiety had an effect on depression.Married patients were less likely to have depressive symptoms than unmarried, divorced or widowed patients.Married patients were 0.184 times more likely to develop depressive symptoms than unmarried, divorced or widowed patients.Patients with anxiety were 9.914 times more likely to develop depressive symptoms than those without anxiety.
    ConclusionThe marital status and anxiety of type 2 diabetes mellitus patients are the influencing factors of depressive state in T2DM patients.The unmarried, divorced or widowed T2DM patients are prone to depressive symptoms.The patients with type 2 diabetes mellitus complicated with anxiety are also prone to depressive mood, which is the focus of psychological intervention.
