IARC对结直肠癌筛查方式评估的解读

IARC′s interpretation on methods of colorectal cancer screening

  • 摘要: 国际癌症研究所(IARC)通过对目前结直肠癌筛查方式的综述回顾,对目前广泛使用的粪便检测、内镜检查和CT结肠成像术对降低结直肠癌发病、死亡的效果及其收益/伤害比进行了综合的评估。认为通过任何形式的粪便隐血检测,降低结直肠癌死亡率的证据充足。但是,降低发病率的效果证据有限;单次内镜检测降低发病率和死亡率的证据均十分充分。而CT结肠成像检测在降低发病率和死亡率方面的证据均十分有限。上海市自2011年其启动了全市居民大肠癌筛查项目,取得了良好的效果,但是在肠镜的依从性上仍和美国存在较大差距,需要增加人群项目参与度,同时鼓励50岁以上人群至少进行一次肠镜检查,进一步降低上海居民结直肠癌发病和死亡的风险。

     

    Abstract: IARC reviews the current screening methods for colorectal cancer including the stool-based tests, endoscopic techniques, CT colonography and evaluates the outcomes with respect to preventive effects, adverse effects comprehensively.For stool-based tests, it is believed that there is sufficient evidence that it reduces the mortality of colorectal cancer but not much evidence that it reduces the incidence.For endoscopic techniques, there is sufficient evidence that it reduces both the disease morbidity and mortality.For CT colonography, there is not much evidence that it reduces either morbidity or mortality of the disease.Shanghai has launched the residents′ colorectal cancer screening program and achieved effective results since the year 2011.However, there is still a great gap between Shanghai and the United States in terms of compliance with endoscopy, so it is necessary to increase the population participation in the program.People over 50 years of age should be encouraged to at least take one colonoscopy annually so as to further reduce the morbidity and mortality of colorectal cancer among Shanghai residents.
