Effect evaluation on intervention for patients with high Hcy and hyperpiesia in the community

    目的分析H型高血压(伴有血浆同型半胱氨酸升高的原发性高血压)患者社区干预的效果,探索社区H型高血压的防控措施,预防和降低脑卒中的发生率。
    方法采用社区干预试验设计,选取经H型高血压筛查同时为上海市疾病控制信息管理平台在管的高血压患者1 070人,按社区分为干预组569人,对照组501人。干预组采用个性化随访与健康教育干预,对照组采用常规随访管理。比较干预前后两组同型半胱氨酸(Hcy)水平、生活方式、体测指标的变化。
    结果干预后,干预组Hcy、舒张压均值的差值优于对照组,差异有统计学意义(P < 0.05);干预组体质指数、收缩压均值的差值与对照组差异均无统计学意义(P>0.05)。两组干预后在吸烟、饮酒方面的改变差异无统计学意义(P>0.05)。干预后,干预组的新鲜蔬菜食用频次高于干预前,差异有统计学意义(P < 0.05)。
    结论社区干预可以降低H型高血压患者的Hcy水平,饮食结构的改善是非药物干预防控H型高血压的有效措施。

     

    Abstract:
    ObjectiveTo analyze the effectiveness of intervention over patients with HHcy and explore preventive practices, so as to prevent and reduce the incidence of stroke in the community.
    MethodsThe study was a random control design.A total of 1 070 participants with HHcy were recruited from the Huangpu District chronic disease management database.Participants were randomly assigned into intervention group (569 persons), which had personal follow-up and health education intervention.Meanwhile, the control group (501persons) received only personal follow-up.Then the levels of Hcy, lifestyle and body index before and after the intervention were compared between the two groups.
    ResultsThe average Hcy value was significantly lower in intervention group compared to that in control group(P < 0.05);but other indexes like BMI, SBP, DBP showed no significant difference between the groups(P>0.05).There were no changes over smoking and drinking rates in both groups(P>0.05).While the intake of vegetable increased in the intervention group(P < 0.05).
    ConclusionThe community-based intervention has positive impact on lowering Hcy value among patients with HHcy.Especially improvement of diet structure is an effective way to prevent HHcy.
