Exploration and research on public health system in new era

  • 摘要: “没有全民健康,就没有全面小康”!国情使然,我国实现全民健康面临的挑战更多,建设一流公共卫生体系是必由之路。2015年始,项目组在上海市加强公共卫生体系建设三年行动计划资助下,汇聚政学研用专家,围绕建设一流体系亟需回答的理论与实践难题展开了探索。主要结果如下:①明确了何为公共卫生(97.6%~99.2%认可);②明确了何为公共卫生体系(99.1%认可);③确认了政府应关注的11个领域48个类型222个公共卫生问题(88.2%~100.0%认可);④形成了一流体系应涵盖的8个要素63个定位(88.8%~93.9%认可),回答了“一流体系应该是什么样”;⑤构建了包含83个指标的评价标准(94.1%~100.0%认可),回答了“如何评价体系是否达到一流”;⑥明确了上海的优势与发展重点。传染病预防控制等5个领域的平均适宜程度评分上海比纽约高15.9%,与一流标准相差27.3%,政府主导下工作成效彰显;未来5~10年需要在形成与完善各司其职、协作联动的机制和资源配置的制度化保障,建立稳定的专业胜任、激励有效的队伍等方面取得突破。

     

    Abstract: The goal of an overall well-off society cannot be achieved without the health of all people. Building a top-level public health system would be the best choice as China is faced with more challenges on the way to "health for all people". Supported by the Fourth Round of the Three-Year Public Health Action Plan in Shanghai (2015-2017), this study has brought together experts from governments, universities, institutes and agencies to conduct research on the theoretical and practical issues on building a top-level public health system. The achievements were as follows: (1) What is Public Health was defined, with the approval rate of 97.6%-99.2%. (2) The definition of Public Health System was clarified, with the approval rate of 99.1%. (3) The public health issues which all governments should pay attention to were demonstrated, including 11 categories, 48 sub-categories and 222 specific issues, with the approval rate of 88.2%-100.0%. (4) What a top-level public health system should be was illustrated, including 8 elements and 63 characteristics, with the approval rate of 88.8%-93.9%. (5) The assessment criteria consisting of 83 indicators were designed, with the approval rate of 94.1%-100.0%, which was used to evaluate the extent to which the public health system has achieved. (6) The advantages and strategies in the public health system of Shanghai were demonstrated. Shanghai's public health system has achieved obvious improvements with the great emphasis of the governments. The performance score of Shanghai was 15.9% higher than that of New York, while there remained a 27.3% gap in being top-level. Future challenges include establishing a coordination mechanism with clarity responsibilities, improving a sound resource allocation mechanism, and retaining a stable, competent, incentive public health workforce.
