• Objective To provide foundation for improving the management of DM patients by monitoring and evaluating the status of blood glucose control among DM patients for two years in a community of Shanghai.Methods From September 2006 to September 2008, the doctors of a community health service center established the archives of the DM patients step by step, did regular follow- up, tested blood glucose levels, evaluated situation of blood glucose control, analyzed the influencing factors of blood glucose and did health education and intervention to the DM patients in this community of Shanghai.Results The number of archives of DM patients increased from 105 in September 2006 to 282 in September 2008 in this community of Shanghai. Nearly 30 percent of DM patients have family history. Regular evaluation of well controlled rate of blood glucose among DM patients was 61.91%, 81. 91%, 75.88%, 74.85% and 73.04% respectively. 82.26% DM patients had their diet completely meet the recom- mendation of doctors.Conclusion The diabetes prevention and control work of this community is in good conditions. The blood glucose level in DM patients is comparatively well controlled.
