Exploration of digital management model in strengthening residents′ health management

Along with the introduction and promotion of the national health reform policy, general practitioners(GP) are to undertake the responsibility for community residents as their "health gatekeeper"in terms of the whole population, the whole life cycle, continuous health management becoming the focus of a new round of development of community health service.The application of informatization technology in this round of medical reform gradually shows its powerful advantages.Through Liangcheng community health service center in the pilot implementation of Shanghai community comprehensive change process, the digital management mode in the exploration of old-age residents health management service process and actual application, practice and thinking were made in this regard so as to achieve universal health national strategy by providing references.
