Abstract:
Shanghai municipal administration of market supervision focuses on "the people's city built by the people, and the people's city for the people". Through the regular "you order, I inspect" activities on food safety the administration has established an important window for disseminating market regulation information, showcasing the image of market regulation, and telling the story of market regulation. Over the past three years, by combining the needs of the citizens in Shanghai and the routine food safety spot-checking plan through the food safety “you order,I inspect” activities, the administration has enabled the public to perceive and evaluate the quality of the inspection services, and has actively built a food safety social co-governance service brand. While ensuring that people can eat at ease and buy at will, we will also boost consumer confidence and promote high-quality development of food safety, so as to create a vivid picture of people participating in and benefiting from food safety governance.

Abstract:
ObjectiveAn experimental method was established to determine the level of trans-fatty acids in edible vegetable oil and the changes of trans-fatty acid content during high-temperature simulated cooking. MethodsFour common edible vegetable oils on the market were selected, and three temperature points were set at 180 ℃ (light stir-fry), 220 ℃ (fry) and 260 ℃ (stir fry). The samples were taken after the heat treatment of 0, 1, 2, 3, 4, 5, 6 and 7 hours,respectively, to investigate the changes of trans-fatty acid content. ResultsUnder the heat treatment of 180 ℃ and 220 ℃, the content of trans-fatty acids remained basically stable in rapeseed oil, corn oil, soybean oil and olive oil. Under the heating condition of 260 ℃, the content of trans-fatty acids in these four vegetable oils increased significantly, and the absolute content of trans-fatty acids in rapeseed oil was the highest, reaching 6.84%. Soybean oil showed the larger increase, up to 247.90% from the initial content. The content of trans-fatty acids in olive oil increased from 0.26% to 1.69%, although the increase was large, but the absolute content of trans-fatty acids was still low. The content of trans-fatty acids in sesame oil increased significantly under the three temperatures, reaching 4.79% when heated at 260 ℃, and an increase of 369.61% compared with no heating. ConclusionTrans-fatty acid content in edible vegetable oils increases significantly in the cooking process, the higher the temperature and the longer the cooking time, the more significant the increase. It is recommended to avoid high temperature cooking and long time fried food.

Abstract:
ObjectiveTo investigate the risk of dietary lead exposure in residents aged 15 years and above in Shanghai. MethodsBased on the dietary consumption survey of residents aged 15 years and above in spring 2012 and the monitoring data of food lead contamination in Shanghai from 2020 to 2021, the dietary exposure level was assessed using the point assessment method and compared with the provisional weekly tolerable intake (PTWI) to assess the exposure risk. ResultsThe overall detection rate of lead in 9 commonly consumed food categories by residents in Shanghai was 56.82%, with a total excess rate of 0.23%. The average weekly dietary lead exposure (by body weight) of residents was 2.979 4 μg·kg⁻¹. The top three dietary lead contributing factors were vegetables (30.69%), cereals (28.88%), and aquatic products (11.12%). The average weekly dietary lead exposure (by body weight) of residents in urban, suburban, and rural areas were 3.282 5, 2.979 5 and 2.478 0 μg·kg⁻¹, respectively. The food with the highest contribution rate of dietary lead exposure in these regions was vegetables, with contribution rates of 33.65%, 29.69%, and 25.22%, respectively. The average weekly dietary lead exposure (by body weight) of male and female residents was 3.077 7 and 2.881 5 μg·kg⁻¹, respectively, with vegetables being the main source of dietary lead, contributing 30.03% and 31.39%, respectively. The average weekly dietary lead exposure (by body weight) of residents aged 15‒, 45‒, and ≥60 years old was 3.084 2, 3.009 9 and 2.874 4 μg·kg⁻¹, respectively. The food with the highest contribution rate of dietary lead exposure in these groups was vegetables, contributing 27.12%, 32.10%, and 32.26%, respectively. ConclusionThe dietary lead exposure of Shanghai residents aged 15 years and above is at an acceptable level, and the vegetables, cereals, and aquatic products with high contribution rates need to be supervised more closely.

Abstract:
ObjectiveWe conducted a two-sample Mendelian randomization (MR) study to assess the causal relationship between circulating isoleucine, leucine and valine levels and the risk of peripheral atherosclerosis. MethodsBased on the large-scale genome-wide association study (GWAS) database, single nucleotide polymorphisms (SNPs) closely related to the circulating levels of isoleucine, leucine and valine were identified as instrumental variables (IVs). Two-sample MR analysis applying the inverse variance weighted (IVW) method and the weighted median estimator (WME) method were performed to estimate the causal relationship between the risk of peripheral atherosclerosis and the exposure with more than three SNPs that were available as IVs. The pleiotropy was evaluated by using the MR-Egger regression and MR-PRESSO method, and the leave-one-out method was used in sensitivity analysis. ResultsFour, one and one SNPs were identified as IVs for circulating isoleucine, leucine and valine levels, respectively. For isoleucine, the IVW model demonstrated there was no evidence of heterogeneity among the IVs (P=0.715), and there was a significant causal relationship between the increase of circulating isoleucine level and a higher risk of peripheral atherosclerosis risk. Per every 1 elevated standard deviation (SD) of circulating isoleucine level resulted in increasing 31% of peripheral atherosclerosis risk (OR=1.31, 95%CI: 1.07‒1.61). Similarly, the OR(95%CI) was 1.33 (1.04‒1.71) in the WME model. The MR-Egger regression and MR-PRESSO analysis indicated no evidence of pleiotropy in IVs (all P>0.05). The result of the leave-one-out sensitivity analysis was stable. The Wald ratio model displayed that the causal relationship between circulating leucine and valine levels and the risk of peripheral atherosclerosis was not statistically significant. The OR (95%CI) for leucine and valine was 1.13 (0.78‒1.63) and 1.11 (0.82‒1.50), respectively. ConclusionThere is a significant causal relationship between the increase of circulating isoleucine level and a higher peripheral atherosclerosis risk. The causal relationships between circulating leucine and valine levels and the risk of peripheral atherosclerosis need to be further confirmed in future studies.

Abstract:
ObjectiveTo investigate the contamination status, serotype distribution and drug resistance of Salmonella in retail poultry in Jiading District, Shanghai. MethodsFour types of poultry (chicken, duck, goose and pigeon) have been sampled from commercial markets. Salmonella contamination has been isolated and identified using serotype analysis. Furthermore, resistance of isolated Salmonella strains towards 16 commonly used antibiotics has been determined. ResultsA total of 60 Salmonella strains were isolated from 80 poultry samples. The detection rates of Salmonella in pigeon, goose, duck and chicken were 86.67%, 81.82%, 72.73% and 59.38%, respectively. Contamination status has been categorized by storage conditions. Lowest detection rate (65.63%) has been noted in poultry samples under refrigeration storage. The majority serotypes of Salmonella have been revealed as Salmonella typhimurium, Salmonellaenteritidis and Salmonella corvallis. Relatively higher drug resistance was discovered with tetracycline, nalidixic acid, ampicillin and chloramphenicol, with drug resistance rate of 60.00%, 58.33%, 50.00% and 48.33%, respectively. Low drug resistance was revealed with cefotaxime. In addition, these Salmonella strains were completely sensitive to imipenem. Significant difference in drug resistance was identified across the types of poultry or Salmonella serotypes. The 11.67% of Salmonella strains were non-resistant to any tested drugs. Multi-drug resistant strains accounted for 46.67% of isolated strains, which were resistant to 13 different antibiotics. Ampicillin-tetracycline or chloramphenirol-cefazolin drug resistance pattern suggested that the Salmonella strain was multi-drug resistant. ConclusionSalmonella contamination remains high in retail poultry in Jiading District, Shanghai. Drug resistance to antibiotics is increasing. Therefore, monitoring and control of Salmonella should be strengthened.

Abstract:
ObjectiveTo systematically evaluate the safety of interferon β-1a for treatment of corona virus disease 2019 (COVID-19), and to provide references for interferon β-1a's clinical application. MethodsThis study was conducted with the database from US Food and Drug Administration adverse event reporting system (FAERS) from January 1, 2015 to March 31, 2021. Information component (IC) and reporting odds ratio (ROR) methods were applied for signal mining. ResultsA total of 463 700 records of COVID-19 were selected for analysis, and 45 positive drug adverse event signals were detected. Headache (IC025=1.09, ROR025=2.28), pyrexia (IC025=0.51, ROR025=1.51) and multiple sclerosis relapse (IC025=3.67, ROR025=14.71) were positive adverse events with higher frequency. Autoimmune disorder (IC025=4.42, ROR025=24.03), streptococcal infection (IC025=4.12, ROR025=19.82), and multiple sclerosis relapse (IC025=3.67, ROR025=14.71) were positive adverse events. Acute lung injury, cardio-respiratory arrest and metabolic acidosis were associated with a higher proportion and frequency of death. ConclusionThere are certain safety issues with interferon β-1a in the treatment of COVID-19, and some adverse events with high frequency and high death rate deserve further attention by medical staffs.

Abstract:
ObjectiveTo describe the serological surveillance results of human brucellosis in Ganzhou City, Jiangxi Province from 2018 to 2021, characterize the epidemic and current situation of brucellosis, and to provide scientific evidence for effective prevention and control of brucellosis. MethodsSurveillance data of human brucellosis serological testing was collected in Ganzhou City from 2018 to 2021. Spatial, temporal, and demographic distribution was further determined. ResultsFrom 2018 to 2021, a total of 42 humans serologically positive for brucellosis were reported from 18 counties (cities and districts) in Ganzhou City, including 26 males and 16 females with a gender ratio of 1.6∶1. The number of serologically positive cases showed a decreasing trend, with the positive rate decreasing from 46.43% in 2018 to 33.33% in 2021. Majority of the cases (54.00%) aged 40‒ years old. Furthermore, the number of serologically positive cases varied by month; majority of the cases (80.95%) was from April to August. Additionally, a total of 10 counties (cities and districts) reported serologically positive cases of brucellosis, among which the top 3 counties (cities and districts) by cumulative number of positive cases were Zhanggong District (18 cases), Dayu County(5 cases), and Longnan City(4 cases). ConclusionSerologically positive cases of human brucellosis decrease in Ganzhou, in which the incidence of male cases is generally higher than female cases. The seasonality of human brucellosis is in spring and summer. At-risk population is 40‒ years old. Additionally, at-risk areas are southwest and central areas of Ganzhou.

Abstract:
ObjectiveTo determine the epidemic characteristics of anthrax in Chengde City, Hebei Province from 2005 to 2021, and to provide evidence for formulating prevention and control measures of anthrax and reducing incidence rate. MethodsThis study collected the data of anthrax epidemic in Chengde City and conducted descriptive analysis. ResultsFrom 2005 to 2021, a total of 11 anthrax cases were reported in Chengde City with no death. The average incidence rate was 0.08/105, which remained low. Furthermore, 10 cases were cutaneous anthrax and 1 case was pulmonary anthrax. The cases were mainly reported in Weichang County, accounting for 90.91% of the total reported cases, followed by Pingquan City. In addition, the cases were mainly reported from July to August and mainly between 30 and 59 years old with a gender ratio of 2.67∶1. ConclusionThe anthrax epidemic in Chengde City is likely to increase. It mainly occurs in summer, rural areas, and male young and middle-aged farmers. It is necessary to improve epidemic monitoring, health education, disease prevention capacity, early identification of the epidemic, and active response.

Abstract:
ObjectiveTo identify causal factors of a case of severe Chlamydia psittaci pneumonia in Yangpu District and provide a scientific basis for effective prevention and control. MethodsBasic information and epidemiological data of the patient were collected through telephone interviews and field epidemiological surveys. Specimens from the patient, close contacts and the environment were collected for pathogen detection. Metagenomics next-generation sequencing (mNGS) was used to identify unknown pathogens. ResultsA 65-year-old male patient with a history of hypertension and diabetes was admitted to the hospital with symptoms of fatigue, poor appetite for a week, fever and cough for four days. A chest computer tomography (CT) scan showed scattered inflammation in the left lung with infiltration of multiple lobes. Blood gas analysis showed type I respiratory failure. The results of mNGS on the bronchoalveolar lavage fluid of the patient indicated that he was infected with Chlamydia psittaci. Epidemiological investigation showed a clear history of avian exposure, with an incubation period of 30 days. ConclusionThis serious pneumonia is a zoonotic disease caused by Chlamydia psittaci. A clear history of avian exposure and the use of mNGS technology can help in the timely diagnosis of this disease.

Abstract:
ObjectiveTo investigate the risk factors of fertility behaviors with preterm birth and low birth weight, and to develop a nomogram model to predict the occurrence of low birth weight. MethodsBirth registration information in Shanghai from 2010 to 2020 was collected, and ANOVA and Chi-square tests were used to compare the differences in reproductive behavior factors and newborn health status across time. The odds ratio (OR) value and 95%CI were calculated by a multi-classification logistic regression model to determine the association between reproductive behavior factors and preterm birth or low birth weight infants. A nomogram model was established based on logistic model and the area under the ROC curve was used to assess the effect of the model. ResultsThis analysis included 2 089 384 live newborns. The incidence of full-term low birth weight, preterm normal weight and preterm low birth weight in Shanghai was 0.94%, 2.48% and 2.01%, respectively. From 2010 to 2020, 40.00% women had a history of abortion, the proportion of women who gave birth at age ≥40 years old increased from 1.05% to 2.24%, the proportion of fathers aged ≥40 years increased from 4.79% to 7.48%, and the proportion of women with postgraduate or above increased from 4.81% to 11.74%. The incidence of preterm low birth weight in Shanghai showed an increasing trend over time. Logistic regression analysis showed that the risk of preterm low birth weight was lower in female than in male infants (OR=0.97, 95%CI: 0.95‒0.98), and the risk of full-term low birth weight was higher than in male infants (OR=1.85, 95%CI: 1.80‒1.90). The risk of preterm birth and low birth weight was lower for couples of childbearing age with higher education. The risk of preterm low birth weight in newborns tended to increase with maternal age at childbirth >30 years, paternal age ≥40 years, and the number of abortions >2 times. Mother <25 or >35 years, father aged 30‒34 years, and the number of abortions >3 times were the risk factors of full-term low birth weight infants. ConclusionCouples of childbearing age who choose to have children at too high or too low age may increase the risk of preterm birth or low birth weight, so it is necessary to strengthen population awareness and promote age-appropriate childbirth. Multiple abortions are also associated with preterm birth and low birth weight, and it is advisable to popularize the scientific knowledge of contraception and birth control to reduce unnecessary abortions. The nomogram in the study can visualize the risk of full-term and low birth weight infant at different levels of factors, which can assist couples preparing for pregnancy in making decisions about the timing of childbirth and understanding the level of risk.

Abstract:
ObjectiveTo compare the incidence of respiratory and allergic diseases in children in Xuhui District, Shanghai in 2013 and 2020, and to determine the influencing factors. MethodsAnnual average levels of air pollutants including PM2.5, PM10, O3, SO2, and NO2 were collected and described in Shanghai from 2013 to 2020. A cross-sectional survey was conducted by using a questionnaire among grade 3 to 5 students in a school in Xuhui District, Shanghai, in September 2013 and 2020, respectively. The questionnaire collected variables including living environment, daily habits, family history of respiratory and allergic diseases, and incidence of these diseases in children. Chi-square test was used to determine the difference across respiratory and allergic diseases. Logistic regression was conducted to determine the influencing factors. ResultsA total of 1 398 valid questionnaires were collected (705 in 2013 and 693 in 2020). Compared with 2013, annual average concentrations of PM2.5, PM10, O3, SO2, and NO2 in 2020 significantly decreased. The prevalence of bronchial asthma, bronchitis, persistent cough and persistent expectoration in 2013 were significantly higher than those in 2020 (P<0.05) in Xuhui District. Multivariate analysis showed that severe air pollution, boys, parents with asthma or allergy, parents with higher educational levels, and more allergens in household were the risk factors associated with the incidence of bronchial asthma, bronchitis, allergic rhinitis and atopic eczema (P<0.05). Parents with allergy history, high smoking frequency of family member, and more allergens in household were the risk factors associated with the incidence of persistent cough and persistent expectoration (P<0.05). ConclusionTo 2013,2020 air pollution in Shanghai has been mitigated and prevalence of bronchial asthma and bronchitis of children has decreased. Childhood respiratory and allergic diseases are associated with indoor and outdoor environment, family medical history, and family daily habits.

Abstract:
ObjectiveTo analyze the effect of O3 pollution on outpatient visits for respiratory diseases in a district of Shanghai. MethodsWe collected the respiratory disease outpatient data, and atmospheric and meteorological data of from a suburban general hospital in Shanghai from 2015 to 2017. A time-series analysis by generalized additive model was conducted to examine the relationship between O3 pollution and daily outpatient visits. ResultsThe daily outpatient volume for respiratory diseases was 831. The daily 8 h median concentration of O3 was 101.04 μg·m-3. The excess relative risk was 0.461% (95%CI: 0.240%‒0.682%) at lag3. Stratified by gender and age, females, child and the aged had higher risk of respiratory diseases. In the double-pollutant model, PM2.5 and PM10 increased health effects, while CO reduced health effects. ConclusionThe increase of O3 concentration can increase the daily outpatient volume of respiratory diseases.

Abstract:
ObjectiveTo explore the effect of mint juice on the nitrite content in pickled cabbage, and to determine the best concentration of mint juice through comprehensive sensory evaluation. MethodsThe control variates method was used to conduct this single factor experiment. The pickled cabbage processing was divided into five groups, one control group CK, and four experimental groups, Lp100, Lp80, Lp60, Lp40, respectively. The data were analyzed by using test of normality, homogeneity of variance test, ANOVA and multiple comparisons. ResultsThe content of nitrite in the experimental group with mint juice was much lower than that in the control group. During each day, as the concentration of mint juice increased, the removal efficiency also increased and the nitrite content in pickled cabbage decreased, too. When the concentration of mint juice was 100%, the nitrite content in pickled cabbage remained the lowest during 1-5 days of fermentation. On the fifth day of fermentation, the removal efficiency of nitrite in pickled cabbage by mint juice with 100% concentration reached the highest (49.55%). Adding 60% mint juice to pickled cabbage showed the highest score of 92 in sensory evaluation. ConclusionMint juice can reduce the nitrite content in pickled cabbage and make pickled cabbage a better flavor. Result indicates that 60% mint juice is the best concentration for a comprehensive effect.

Abstract:
ObjectiveTo explore the barriers and facilitators of the adherence of formal practice after mindfulness-based cognitive therapy (MBCT) in psychological counseling outpatients. MethodsOne-on-one interview was conducted in 15 psychological counseling outpatients who attended MBCT at Shanghai Mental Health Center and had been out of treatment 5 months or more. Data was analyzed using thematic analysis. ResultsPoor mood, weak willpower, limited time and space, lack of companionship and supervision, and discomfort with the recording were the five factors that prevented the patients from sticking to their practice. Personality trait, trust, benefit, need for self-care, time/space arrangement, fellow practitioners, therapists, and ease and convenience of practice were the eight factors that promoted the patients to practice. ConclusionThe COM-B model helps therapists and individuals with practice needs to understand the mechanism of formal practice facilitators at a holistic level. Therapists and individuals with practice needs should be concerned about possible barriers on the adherence of formal practice.

Abstract:
ObjectiveThis paper analyzed the punishment situation of unlicensed medical practice in recent 7 years in Jinshan District of Shanghai, then suggested the countermeasures to prevent and combat unlicensed medical practice in the future. MethodsA total of 190 cases of unlicensed medical practice from 2016 to 2022 were investigated. The basic situation of the punishment,legal basis,subject distribution,clue source and individual characteristics were analyzed. ResultsThe case-filing number of unlicensed medical practice showed a decreasing trend,but the amount of fines increased obviously. The clues of investigation and punishment of unlicensed medicine practice were mainly found by inspection(32.11%) and transferred from other departments(28.42%), while the number of complaints and clues reported from grassroots increased year by year. Medical and dental were the two major unlicensed practice,accounting for 35.79% and 33.16%, respectively. In addition, unlicensed medical cosmetology practice was increasing year by year. 78.62% of the unlicensed medical practioners are immigrants, 62.76% of them aged between 31 and 50 years old, among them 70.34% are men, and 62.76% of them have a fixed place. ConclusionThough the prevention and crack-down on unlicensed medical practice has achieved significant achievements,it is still necessary to enhance the responsibility of local administration,strengthen the inspection,investigate and deal with cases jointly by multiple departments,and implement the comprehensive supervision mechanism of the health care industry.

With the rapid development of aging around the world, the incidence and prevalence of age-related dementia have increased significantly, leading to a huge burden on patients, families and society. So far, there are no drugs that can completely reverse degeneration of the nervous system. The core pathological mechanisms of dementia are not clear, and the occurrence and progression of degenerative diseases in the nervous system appears to be only rely on the prevention through lifestyle interventions. Among lifestyle interventions, a large amount of laboratory evidence and a small amount of epidemiological evidence suggest that time restricted feeding have excellent effects on preventing the occurrence and progression of degenerative diseases in the nervous system. However, due to the lack of evidence in human population, the application of time restricted feeding lacks scientific basis. By synthesizing the existing evidence, this review discussed the pathway network of time restricted feeding that antagonizes the nervous system degradation, suggesting that future studies should focus on population evidence and the combined effects of multiple effect pathways.

So far, there are few studies on the detection of bisphenol A in self-heating food packaging materials, and there remains a lack of relevant standard. Therefore, it is necessary to establish a simple, sensitive and efficient method for the detection of bisphenol A in self-heating food. Especially in the global context where greater attention is given to disadvantaged groups, there is a need to cater to the needs of the elderly in terms of rehabilitation and assistive aid to improve their quality of life. Compared with developed countries, China’s rehabilitation aid industry is still in its early stages of development and needs to move away from traditional equipment manufacturing and integrate with intelligent manufacturing to provide more development possibilities and choices. International cooperation is expected to become one of the future research and development directions for rehabilitation aids. Taking typical cities and countries at home and abroad as examples, this paper explores the development of rehabilitation aids, and calls for the cultivation of more rehabilitation aid professionals to help more people in need.

Abstract:
ObjectiveAn experimental method was established to determine the level of trans-fatty acids in edible vegetable oil and the changes of trans-fatty acid content during high-temperature simulated cooking. MethodsFour common edible vegetable oils on the market were selected, and three temperature points were set at 180 ℃ (light stir-fry), 220 ℃ (fry) and 260 ℃ (stir fry). The samples were taken after the heat treatment of 0, 1, 2, 3, 4, 5, 6 and 7 hours,respectively, to investigate the changes of trans-fatty acid content. ResultsUnder the heat treatment of 180 ℃ and 220 ℃, the content of trans-fatty acids remained basically stable in rapeseed oil, corn oil, soybean oil and olive oil. Under the heating condition of 260 ℃, the content of trans-fatty acids in these four vegetable oils increased significantly, and the absolute content of trans-fatty acids in rapeseed oil was the highest, reaching 6.84%. Soybean oil showed the larger increase, up to 247.90% from the initial content. The content of trans-fatty acids in olive oil increased from 0.26% to 1.69%, although the increase was large, but the absolute content of trans-fatty acids was still low. The content of trans-fatty acids in sesame oil increased significantly under the three temperatures, reaching 4.79% when heated at 260 ℃, and an increase of 369.61% compared with no heating. ConclusionTrans-fatty acid content in edible vegetable oils increases significantly in the cooking process, the higher the temperature and the longer the cooking time, the more significant the increase. It is recommended to avoid high temperature cooking and long time fried food.

Abstract:
ObjectiveTo investigate the risk of dietary lead exposure in residents aged 15 years and above in Shanghai. MethodsBased on the dietary consumption survey of residents aged 15 years and above in spring 2012 and the monitoring data of food lead contamination in Shanghai from 2020 to 2021, the dietary exposure level was assessed using the point assessment method and compared with the provisional weekly tolerable intake (PTWI) to assess the exposure risk. ResultsThe overall detection rate of lead in 9 commonly consumed food categories by residents in Shanghai was 56.82%, with a total excess rate of 0.23%. The average weekly dietary lead exposure (by body weight) of residents was 2.979 4 μg·kg⁻¹. The top three dietary lead contributing factors were vegetables (30.69%), cereals (28.88%), and aquatic products (11.12%). The average weekly dietary lead exposure (by body weight) of residents in urban, suburban, and rural areas were 3.282 5, 2.979 5 and 2.478 0 μg·kg⁻¹, respectively. The food with the highest contribution rate of dietary lead exposure in these regions was vegetables, with contribution rates of 33.65%, 29.69%, and 25.22%, respectively. The average weekly dietary lead exposure (by body weight) of male and female residents was 3.077 7 and 2.881 5 μg·kg⁻¹, respectively, with vegetables being the main source of dietary lead, contributing 30.03% and 31.39%, respectively. The average weekly dietary lead exposure (by body weight) of residents aged 15‒, 45‒, and ≥60 years old was 3.084 2, 3.009 9 and 2.874 4 μg·kg⁻¹, respectively. The food with the highest contribution rate of dietary lead exposure in these groups was vegetables, contributing 27.12%, 32.10%, and 32.26%, respectively. ConclusionThe dietary lead exposure of Shanghai residents aged 15 years and above is at an acceptable level, and the vegetables, cereals, and aquatic products with high contribution rates need to be supervised more closely.

Abstract:
ObjectiveWe conducted a two-sample Mendelian randomization (MR) study to assess the causal relationship between circulating isoleucine, leucine and valine levels and the risk of peripheral atherosclerosis. MethodsBased on the large-scale genome-wide association study (GWAS) database, single nucleotide polymorphisms (SNPs) closely related to the circulating levels of isoleucine, leucine and valine were identified as instrumental variables (IVs). Two-sample MR analysis applying the inverse variance weighted (IVW) method and the weighted median estimator (WME) method were performed to estimate the causal relationship between the risk of peripheral atherosclerosis and the exposure with more than three SNPs that were available as IVs. The pleiotropy was evaluated by using the MR-Egger regression and MR-PRESSO method, and the leave-one-out method was used in sensitivity analysis. ResultsFour, one and one SNPs were identified as IVs for circulating isoleucine, leucine and valine levels, respectively. For isoleucine, the IVW model demonstrated there was no evidence of heterogeneity among the IVs (P=0.715), and there was a significant causal relationship between the increase of circulating isoleucine level and a higher risk of peripheral atherosclerosis risk. Per every 1 elevated standard deviation (SD) of circulating isoleucine level resulted in increasing 31% of peripheral atherosclerosis risk (OR=1.31, 95%CI: 1.07‒1.61). Similarly, the OR(95%CI) was 1.33 (1.04‒1.71) in the WME model. The MR-Egger regression and MR-PRESSO analysis indicated no evidence of pleiotropy in IVs (all P>0.05). The result of the leave-one-out sensitivity analysis was stable. The Wald ratio model displayed that the causal relationship between circulating leucine and valine levels and the risk of peripheral atherosclerosis was not statistically significant. The OR (95%CI) for leucine and valine was 1.13 (0.78‒1.63) and 1.11 (0.82‒1.50), respectively. ConclusionThere is a significant causal relationship between the increase of circulating isoleucine level and a higher peripheral atherosclerosis risk. The causal relationships between circulating leucine and valine levels and the risk of peripheral atherosclerosis need to be further confirmed in future studies.

Abstract:
ObjectiveTo investigate the contamination status, serotype distribution and drug resistance of Salmonella in retail poultry in Jiading District, Shanghai. MethodsFour types of poultry (chicken, duck, goose and pigeon) have been sampled from commercial markets. Salmonella contamination has been isolated and identified using serotype analysis. Furthermore, resistance of isolated Salmonella strains towards 16 commonly used antibiotics has been determined. ResultsA total of 60 Salmonella strains were isolated from 80 poultry samples. The detection rates of Salmonella in pigeon, goose, duck and chicken were 86.67%, 81.82%, 72.73% and 59.38%, respectively. Contamination status has been categorized by storage conditions. Lowest detection rate (65.63%) has been noted in poultry samples under refrigeration storage. The majority serotypes of Salmonella have been revealed as Salmonella typhimurium, Salmonellaenteritidis and Salmonella corvallis. Relatively higher drug resistance was discovered with tetracycline, nalidixic acid, ampicillin and chloramphenicol, with drug resistance rate of 60.00%, 58.33%, 50.00% and 48.33%, respectively. Low drug resistance was revealed with cefotaxime. In addition, these Salmonella strains were completely sensitive to imipenem. Significant difference in drug resistance was identified across the types of poultry or Salmonella serotypes. The 11.67% of Salmonella strains were non-resistant to any tested drugs. Multi-drug resistant strains accounted for 46.67% of isolated strains, which were resistant to 13 different antibiotics. Ampicillin-tetracycline or chloramphenirol-cefazolin drug resistance pattern suggested that the Salmonella strain was multi-drug resistant. ConclusionSalmonella contamination remains high in retail poultry in Jiading District, Shanghai. Drug resistance to antibiotics is increasing. Therefore, monitoring and control of Salmonella should be strengthened.

Abstract:
ObjectiveTo systematically evaluate the safety of interferon β-1a for treatment of corona virus disease 2019 (COVID-19), and to provide references for interferon β-1a's clinical application. MethodsThis study was conducted with the database from US Food and Drug Administration adverse event reporting system (FAERS) from January 1, 2015 to March 31, 2021. Information component (IC) and reporting odds ratio (ROR) methods were applied for signal mining. ResultsA total of 463 700 records of COVID-19 were selected for analysis, and 45 positive drug adverse event signals were detected. Headache (IC025=1.09, ROR025=2.28), pyrexia (IC025=0.51, ROR025=1.51) and multiple sclerosis relapse (IC025=3.67, ROR025=14.71) were positive adverse events with higher frequency. Autoimmune disorder (IC025=4.42, ROR025=24.03), streptococcal infection (IC025=4.12, ROR025=19.82), and multiple sclerosis relapse (IC025=3.67, ROR025=14.71) were positive adverse events. Acute lung injury, cardio-respiratory arrest and metabolic acidosis were associated with a higher proportion and frequency of death. ConclusionThere are certain safety issues with interferon β-1a in the treatment of COVID-19, and some adverse events with high frequency and high death rate deserve further attention by medical staffs.

Abstract:
ObjectiveTo describe the serological surveillance results of human brucellosis in Ganzhou City, Jiangxi Province from 2018 to 2021, characterize the epidemic and current situation of brucellosis, and to provide scientific evidence for effective prevention and control of brucellosis. MethodsSurveillance data of human brucellosis serological testing was collected in Ganzhou City from 2018 to 2021. Spatial, temporal, and demographic distribution was further determined. ResultsFrom 2018 to 2021, a total of 42 humans serologically positive for brucellosis were reported from 18 counties (cities and districts) in Ganzhou City, including 26 males and 16 females with a gender ratio of 1.6∶1. The number of serologically positive cases showed a decreasing trend, with the positive rate decreasing from 46.43% in 2018 to 33.33% in 2021. Majority of the cases (54.00%) aged 40‒ years old. Furthermore, the number of serologically positive cases varied by month; majority of the cases (80.95%) was from April to August. Additionally, a total of 10 counties (cities and districts) reported serologically positive cases of brucellosis, among which the top 3 counties (cities and districts) by cumulative number of positive cases were Zhanggong District (18 cases), Dayu County(5 cases), and Longnan City(4 cases). ConclusionSerologically positive cases of human brucellosis decrease in Ganzhou, in which the incidence of male cases is generally higher than female cases. The seasonality of human brucellosis is in spring and summer. At-risk population is 40‒ years old. Additionally, at-risk areas are southwest and central areas of Ganzhou.

Abstract:
ObjectiveTo determine the epidemic characteristics of anthrax in Chengde City, Hebei Province from 2005 to 2021, and to provide evidence for formulating prevention and control measures of anthrax and reducing incidence rate. MethodsThis study collected the data of anthrax epidemic in Chengde City and conducted descriptive analysis. ResultsFrom 2005 to 2021, a total of 11 anthrax cases were reported in Chengde City with no death. The average incidence rate was 0.08/105, which remained low. Furthermore, 10 cases were cutaneous anthrax and 1 case was pulmonary anthrax. The cases were mainly reported in Weichang County, accounting for 90.91% of the total reported cases, followed by Pingquan City. In addition, the cases were mainly reported from July to August and mainly between 30 and 59 years old with a gender ratio of 2.67∶1. ConclusionThe anthrax epidemic in Chengde City is likely to increase. It mainly occurs in summer, rural areas, and male young and middle-aged farmers. It is necessary to improve epidemic monitoring, health education, disease prevention capacity, early identification of the epidemic, and active response.

Abstract:
ObjectiveTo identify causal factors of a case of severe Chlamydia psittaci pneumonia in Yangpu District and provide a scientific basis for effective prevention and control. MethodsBasic information and epidemiological data of the patient were collected through telephone interviews and field epidemiological surveys. Specimens from the patient, close contacts and the environment were collected for pathogen detection. Metagenomics next-generation sequencing (mNGS) was used to identify unknown pathogens. ResultsA 65-year-old male patient with a history of hypertension and diabetes was admitted to the hospital with symptoms of fatigue, poor appetite for a week, fever and cough for four days. A chest computer tomography (CT) scan showed scattered inflammation in the left lung with infiltration of multiple lobes. Blood gas analysis showed type I respiratory failure. The results of mNGS on the bronchoalveolar lavage fluid of the patient indicated that he was infected with Chlamydia psittaci. Epidemiological investigation showed a clear history of avian exposure, with an incubation period of 30 days. ConclusionThis serious pneumonia is a zoonotic disease caused by Chlamydia psittaci. A clear history of avian exposure and the use of mNGS technology can help in the timely diagnosis of this disease.

Abstract:
ObjectiveTo investigate the risk factors of fertility behaviors with preterm birth and low birth weight, and to develop a nomogram model to predict the occurrence of low birth weight. MethodsBirth registration information in Shanghai from 2010 to 2020 was collected, and ANOVA and Chi-square tests were used to compare the differences in reproductive behavior factors and newborn health status across time. The odds ratio (OR) value and 95%CI were calculated by a multi-classification logistic regression model to determine the association between reproductive behavior factors and preterm birth or low birth weight infants. A nomogram model was established based on logistic model and the area under the ROC curve was used to assess the effect of the model. ResultsThis analysis included 2 089 384 live newborns. The incidence of full-term low birth weight, preterm normal weight and preterm low birth weight in Shanghai was 0.94%, 2.48% and 2.01%, respectively. From 2010 to 2020, 40.00% women had a history of abortion, the proportion of women who gave birth at age ≥40 years old increased from 1.05% to 2.24%, the proportion of fathers aged ≥40 years increased from 4.79% to 7.48%, and the proportion of women with postgraduate or above increased from 4.81% to 11.74%. The incidence of preterm low birth weight in Shanghai showed an increasing trend over time. Logistic regression analysis showed that the risk of preterm low birth weight was lower in female than in male infants (OR=0.97, 95%CI: 0.95‒0.98), and the risk of full-term low birth weight was higher than in male infants (OR=1.85, 95%CI: 1.80‒1.90). The risk of preterm birth and low birth weight was lower for couples of childbearing age with higher education. The risk of preterm low birth weight in newborns tended to increase with maternal age at childbirth >30 years, paternal age ≥40 years, and the number of abortions >2 times. Mother <25 or >35 years, father aged 30‒34 years, and the number of abortions >3 times were the risk factors of full-term low birth weight infants. ConclusionCouples of childbearing age who choose to have children at too high or too low age may increase the risk of preterm birth or low birth weight, so it is necessary to strengthen population awareness and promote age-appropriate childbirth. Multiple abortions are also associated with preterm birth and low birth weight, and it is advisable to popularize the scientific knowledge of contraception and birth control to reduce unnecessary abortions. The nomogram in the study can visualize the risk of full-term and low birth weight infant at different levels of factors, which can assist couples preparing for pregnancy in making decisions about the timing of childbirth and understanding the level of risk.

Abstract:
ObjectiveTo compare the incidence of respiratory and allergic diseases in children in Xuhui District, Shanghai in 2013 and 2020, and to determine the influencing factors. MethodsAnnual average levels of air pollutants including PM2.5, PM10, O3, SO2, and NO2 were collected and described in Shanghai from 2013 to 2020. A cross-sectional survey was conducted by using a questionnaire among grade 3 to 5 students in a school in Xuhui District, Shanghai, in September 2013 and 2020, respectively. The questionnaire collected variables including living environment, daily habits, family history of respiratory and allergic diseases, and incidence of these diseases in children. Chi-square test was used to determine the difference across respiratory and allergic diseases. Logistic regression was conducted to determine the influencing factors. ResultsA total of 1 398 valid questionnaires were collected (705 in 2013 and 693 in 2020). Compared with 2013, annual average concentrations of PM2.5, PM10, O3, SO2, and NO2 in 2020 significantly decreased. The prevalence of bronchial asthma, bronchitis, persistent cough and persistent expectoration in 2013 were significantly higher than those in 2020 (P<0.05) in Xuhui District. Multivariate analysis showed that severe air pollution, boys, parents with asthma or allergy, parents with higher educational levels, and more allergens in household were the risk factors associated with the incidence of bronchial asthma, bronchitis, allergic rhinitis and atopic eczema (P<0.05). Parents with allergy history, high smoking frequency of family member, and more allergens in household were the risk factors associated with the incidence of persistent cough and persistent expectoration (P<0.05). ConclusionTo 2013,2020 air pollution in Shanghai has been mitigated and prevalence of bronchial asthma and bronchitis of children has decreased. Childhood respiratory and allergic diseases are associated with indoor and outdoor environment, family medical history, and family daily habits.

Abstract:
ObjectiveTo analyze the effect of O3 pollution on outpatient visits for respiratory diseases in a district of Shanghai. MethodsWe collected the respiratory disease outpatient data, and atmospheric and meteorological data of from a suburban general hospital in Shanghai from 2015 to 2017. A time-series analysis by generalized additive model was conducted to examine the relationship between O3 pollution and daily outpatient visits. ResultsThe daily outpatient volume for respiratory diseases was 831. The daily 8 h median concentration of O3 was 101.04 μg·m-3. The excess relative risk was 0.461% (95%CI: 0.240%‒0.682%) at lag3. Stratified by gender and age, females, child and the aged had higher risk of respiratory diseases. In the double-pollutant model, PM2.5 and PM10 increased health effects, while CO reduced health effects. ConclusionThe increase of O3 concentration can increase the daily outpatient volume of respiratory diseases.

Abstract:
ObjectiveTo explore the effect of mint juice on the nitrite content in pickled cabbage, and to determine the best concentration of mint juice through comprehensive sensory evaluation. MethodsThe control variates method was used to conduct this single factor experiment. The pickled cabbage processing was divided into five groups, one control group CK, and four experimental groups, Lp100, Lp80, Lp60, Lp40, respectively. The data were analyzed by using test of normality, homogeneity of variance test, ANOVA and multiple comparisons. ResultsThe content of nitrite in the experimental group with mint juice was much lower than that in the control group. During each day, as the concentration of mint juice increased, the removal efficiency also increased and the nitrite content in pickled cabbage decreased, too. When the concentration of mint juice was 100%, the nitrite content in pickled cabbage remained the lowest during 1-5 days of fermentation. On the fifth day of fermentation, the removal efficiency of nitrite in pickled cabbage by mint juice with 100% concentration reached the highest (49.55%). Adding 60% mint juice to pickled cabbage showed the highest score of 92 in sensory evaluation. ConclusionMint juice can reduce the nitrite content in pickled cabbage and make pickled cabbage a better flavor. Result indicates that 60% mint juice is the best concentration for a comprehensive effect.

Abstract:
ObjectiveTo explore the barriers and facilitators of the adherence of formal practice after mindfulness-based cognitive therapy (MBCT) in psychological counseling outpatients. MethodsOne-on-one interview was conducted in 15 psychological counseling outpatients who attended MBCT at Shanghai Mental Health Center and had been out of treatment 5 months or more. Data was analyzed using thematic analysis. ResultsPoor mood, weak willpower, limited time and space, lack of companionship and supervision, and discomfort with the recording were the five factors that prevented the patients from sticking to their practice. Personality trait, trust, benefit, need for self-care, time/space arrangement, fellow practitioners, therapists, and ease and convenience of practice were the eight factors that promoted the patients to practice. ConclusionThe COM-B model helps therapists and individuals with practice needs to understand the mechanism of formal practice facilitators at a holistic level. Therapists and individuals with practice needs should be concerned about possible barriers on the adherence of formal practice.

Abstract:
ObjectiveThis paper analyzed the punishment situation of unlicensed medical practice in recent 7 years in Jinshan District of Shanghai, then suggested the countermeasures to prevent and combat unlicensed medical practice in the future. MethodsA total of 190 cases of unlicensed medical practice from 2016 to 2022 were investigated. The basic situation of the punishment,legal basis,subject distribution,clue source and individual characteristics were analyzed. ResultsThe case-filing number of unlicensed medical practice showed a decreasing trend,but the amount of fines increased obviously. The clues of investigation and punishment of unlicensed medicine practice were mainly found by inspection(32.11%) and transferred from other departments(28.42%), while the number of complaints and clues reported from grassroots increased year by year. Medical and dental were the two major unlicensed practice,accounting for 35.79% and 33.16%, respectively. In addition, unlicensed medical cosmetology practice was increasing year by year. 78.62% of the unlicensed medical practioners are immigrants, 62.76% of them aged between 31 and 50 years old, among them 70.34% are men, and 62.76% of them have a fixed place. ConclusionThough the prevention and crack-down on unlicensed medical practice has achieved significant achievements,it is still necessary to enhance the responsibility of local administration,strengthen the inspection,investigate and deal with cases jointly by multiple departments,and implement the comprehensive supervision mechanism of the health care industry.

Abstract:
With the rapid development of aging around the world, the incidence and prevalence of age-related dementia have increased significantly, leading to a huge burden on patients, families and society. So far, there are no drugs that can completely reverse degeneration of the nervous system. The core pathological mechanisms of dementia are not clear, and the occurrence and progression of degenerative diseases in the nervous system appears to be only rely on the prevention through lifestyle interventions. Among lifestyle interventions, a large amount of laboratory evidence and a small amount of epidemiological evidence suggest that time restricted feeding have excellent effects on preventing the occurrence and progression of degenerative diseases in the nervous system. However, due to the lack of evidence in human population, the application of time restricted feeding lacks scientific basis. By synthesizing the existing evidence, this review discussed the pathway network of time restricted feeding that antagonizes the nervous system degradation, suggesting that future studies should focus on population evidence and the combined effects of multiple effect pathways.

However, it is an endocrine disrupting chemical, which has estrogen activity and can cause certain damage to humans. So far, there are few studies on the detection of bisphenol A in self-heating food packaging materials, and there remains a lack of relevant standard. Therefore, it is necessary to establish a simple, sensitive and efficient method for the detection of bisphenol A in self-heating food. This study briefly introduces the pretreatment methods of bisphenol A, such as ultrasonic extraction, solid phase extraction, accelerated solvent extraction, and detection methods, such as gas chromatography-mass spectrometry, high performance liquid chromatography, fluorescent detection, and electrochemical detection.

With a rapidly aging population, there is a huge potential demand for rehabilitation and assistive products and services. Especially in the global context where greater attention is given to disadvantaged groups, there is a need to cater to the needs of the elderly in terms of rehabilitation and assistive aid to improve their quality of life. Compared with developed countries, China’s rehabilitation aid industry is still in its early stages of development and needs to move away from traditional equipment manufacturing and integrate with intelligent manufacturing to provide more development possibilities and choices. International cooperation is expected to become one of the future research and development directions for rehabilitation aids. Taking typical cities and countries at home and abroad as examples, this paper explores the development of rehabilitation aids, and calls for the cultivation of more rehabilitation aid professionals to help more people in need.
