新生儿血清胆红素水平与脑干听觉诱发电位的相关性

Correlation between neonatal serum bilirubin and brainstem auditory evoked potentials

    目的分析新生儿血清胆红素水平与脑干听觉诱发电位(ABR)的相关性。
    方法选择2015年1月至2016年6月入院进行治疗的80例新生儿黄疸患儿,根据入院时血清总胆红素浓度分为A组(35例)、B组(30例)以及C组(15例),并进行ABR检查。
    结果A组患儿血清胆红素为(137~171 μmol/L),ABR异常19例,异常率为54.29%;B组患儿血清胆红素为(172~205 μmol/L),ABR异常28例,异常率为93.33%;C组患儿血清胆红素为(206~257 μmol/L),ABR异常率为100.00%。A组与B、C组的ABR异常率差异有统计学意义(P < 0.05)。全部患儿胆红素水平恢复正常后3个月进行复查,A、B、C组的ABR异常恢复率分别为100.00%、40.00%及0.00%,组间差异有统计学意义(P < 0.05)。
    结论ABR与新生儿血清胆红素浓度呈相关性,当患儿的血清胆红素浓度达到171 μmol/L时,临床治疗就要给予足够重视,避免发展成为永久性中枢神经损伤。

     

    Abstract:
    ObjectiveTo analyze the relationship between neonatal serum bilirubin levels and brainstem auditory evoked potentials (ABR).
    MethodsEighty neonates with neonatal jaundice who were treated in hospital from January 2015 to June 2016 were selected and according to their serum total bilirubin concentrations determined on admission, divided into group A(35 cases), group B(30 cases), group C(15 cases), and brain auditory evoked potentials were determined.
    ResultsIn group A, serum bilirubin (137-171 μmol/L) and abnormal ABR were found in 19 cases, the abnormal rate was 54.29%.In group B, serum bilirubin (172-205 μmol/L) and abnormal ABR were found in 28 cases, the abnormal rate was 93.33%.All the ABRs in group C (206-257 μmol/L) were abnormal. There was a statistically significant difference in the ABR abnormality rate between group A and group B, C(P < 0.05).All children's bilirubin concentration returned to normal 3 months after the re-examination; the recovery rates in group A, group B and group C were 100.00%, 40.00% and 0.00% respectively.The difference between the groups was significant(P < 0.05).
    ConclusionBrainstem auditory evoked potentials are associated with neonatal serum bilirubin concentrations, especially when the serum bilirubin concentration in children reaches 171 μmol/L.Clinical treatment must be given sufficient attention to avoid the development of a permanent central nervous system damage.
