29 902例过敏性疾病患者血清过敏原特异性免疫球蛋白E的检测结果

Serum allergen-specific IgE in 29 902 patients with allergy diseases

    目的分析过敏性疾病患者血清过敏原特异性免疫球蛋白E(IgE)的水平和特征,为临床诊断和治疗提供依据。
    方法收集浙江省杭州地区29 902例过敏性疾病患者,采用酶联免疫法检测血清总IgE(tIgE)和过敏原特异性IgE(sIgE),并分析过敏原sIgE的性别、年龄、时间分布差异和共致敏情况。
    结果29 902例过敏性疾病患者的血清总IgE(tIgE)阳性率为77.34%。吸入性过敏原最常见的为尘螨、豚草和树花粉组合,sIgE阳性率分别为36.75%、11.23%和9.06%;食入性过敏原最常见的为鸡蛋白、牛奶和蟹,sIgE阳性率分别为9.90%、8.88%和4.62%。男性的过敏原sIgE阳性率高于女性;<18岁者的过敏原sIgE阳性率高于≥18岁者;树花粉组合的sIgE阳性率呈逐年下降的趋势。对1种过敏原过敏者最多,占过敏原sIgE阳性患者总数的52.52%。
    结论尘螨、豚草、树花粉组合、鸡蛋白、牛奶是杭州地区重要的过敏原,过敏性疾病的预防和治疗研究工作应当主要围绕这几类过敏原展开;不同年龄、性别的患者过敏原分布不同,防治工作应各有所侧重。

     

    Abstract:
    ObjectiveTo analyze allergen-specific IgE (sIgE) in the serum of patients with allergic diseases, and provide the basis for clinical diagnosis and treatment.
    MethodsThe serum sIgE of 29 902 patients with allergic diseases in Hangzhou City of Zhejiang Province was detected by ELISA, and the result was analyzed with gender, age, time distribution.
    ResultsThe positive rate of total IgE (tIgE) in 29 902 patients with allergic diseases was 77.34%. The most common inhaled allergens were dust mite (36.75%), ragweed (11.23%) and tree pollen (9.06%), and the most common food allergens were egg protein (9.90%), milk (8.88%) and crab (4.62%). The positive rate of sIgE in men was higher than that in women. The positive rate of sIgE in people less than 18 years old was higher than that in people more than 18 years old. The positive rates of tree pollen sIgE decreased year by year. Most patients were allergic to one allergen, accounting for 52.52% of the total sIgE positive patients.
    ConclusionDust mite, ragweed, tree pollens, egg protein and milk are important allergens in Hangzhou. The prevention and treatment of allergic diseases should focus on these allergens. We should understand that the distribution of allergens in different ages and genders is different, so the prevention and treatment should focus on different aspects.
