医院感染和突发公共卫生事件应对能力评估指标构建

Development of assessment indicators for healthcare-associated infection and public health emergency response capacity

    目的 运用德尔菲法构建适用于医院感染预防与控制(简称“感控”)体系的突发公共卫生事件应对能力评估指标,为市级医院感控管理提供依据。
    方法 基于文献分析和理论研究初步构建应对能力指标体系,运用德尔菲法对17名专家进行2轮咨询,采用层次分析法计算指标权重。
    结果 2轮专家咨询的函询问卷有效回收率均为100%,第一轮和第二轮专家权威系数分别为0.97和0.95;肯德尔和谐系数(Kendall’s W)分别为0.31~0.58和0.45~0.58,差异有统计学意义(均P<0.01)。经过2轮专家咨询后,形成市级医院感染防控体系对突发公共卫生事件应对能力的评估指标,包含5个一级指标、18个二级指标及59个三级指标。
    结论 构建的适用于市级医院感控体系的突发公共卫生事件应对能力评估指标具有较强的科学性、可靠性和实用性,为提高市级医院感控能力提供理论基础和价值参考。

     

    Abstract:
    Objective To construct an evaluation index system for public health emergency response capacity within the healthcare-associated infection prevention and control systems using the Delphi method, and to provide a basis for infection control management in municipal hospitals.
    Methods Based on literature analyses and theoretical researches, a preliminary evaluation index system for response capacity was constructed. The Delphi method was employed to conduct two rounds of consultation with 17 experts. The Analytic Hierarchy Process (AHP) was used to calculate the weights of the indicators.
    Results The effective questionnaire return rates for both rounds of expert consultation were 100%. The expert authority coefficients for the first and second rounds were 0.97 and 0.95, respectively. Kendall’s W coefficients ranged from 0.31 to 0.58 in the first round and from 0.45 to 0.58 in the second round, with all differences being statistically significant (P<0.01). After two rounds of expert consultation, the municipal-level hospital infection prevention and control system emergency response capacity evaluation index system was finalized, comprising 5 first-level indicators, 18 second-level indicators and 59 third-level indicators.
    Conclusion The constructed public health emergency response capacity evaluation index system for municipal-level hospital infection prevention and control systems demonstrates strong scientific rigor, reliability, and practicality, providing a theoretical foundation and valuable reference for enhancing the infection prevention and control capacity of municipal-level hospitals.
