德宏傣族景颇族自治州跨境婚姻人群接受艾滋病相关卫生服务现况

Current status of HIV-related health services uptake among cross-border couples in Dehong Prefecture

  • 摘要: 【目的】 了解云南省德宏傣族景颇族自治州(简称“德宏州”)跨境婚姻人群(中国籍丈夫与缅甸籍妻子)艾滋病相关卫生服务利用现况,为优化边境地区艾滋病防控策略提供科学依据。【方法】 2022年9月—2023年9月,采用横断面调查研究方法,对居住在德宏州的14 662对跨境婚姻夫妻进行问卷调查。收集研究对象的社会人口学特征、艾滋病知识知晓情况及近1年是否接受艾滋病相关卫生服务情况等信息。采用多因素logistic回归模型,分析夫妻双方近1年内接受艾滋病检测服务的影响因素。【结果】 研究共纳入29 324人(14 662对夫妻)。总体艾滋病知识知晓率为82.09%,丈夫高于妻子(85.17% vs.79.01%,P<0.001)。94.98%的研究对象至少接受过1项艾滋病相关卫生服务,近1年HIV检测率为65.52%。多因素分析显示,以婚龄0~3年为参照,婚龄4~10年和≥11年者接受HIV检测的OR值分别为:丈夫1.344 (95% CI: 1.187~1.521)和1.381(95% CI: 1.211~1.574),妻子1.308(95% CI: 1.155~1.481)和1.344(95% CI: 1.178~1.533)。以办理结婚证者为参照,持边民通婚备案登记证者的OR值为:丈夫1.710(95% CI: 1.575~1.857),妻子1.700(95% CI: 1.565~1.847);以文盲为参照,小学文化程度者的OR值为:丈夫0.738(95% CI:0.650~0.837),妻子0.827(95% CI: 0.766~0.891)。此外,既往性病史(OR=3.656,95% CI: 1.243~15.589)及参加新型农村合作医疗(OR=1.512,95% CI: 1.179~1.935)是丈夫接受检测的独立相关因素。【结论】 德宏州跨境婚姻人群的艾滋病相关卫生服务利用总体较好,但存在夫妻间差异,相较于中国籍丈夫,外籍妻子的知识知晓率与服务获取程度相对较低。婚龄较短、小学文化程度及未纳入边民通婚备案管理的人群是艾滋病检测服务的薄弱群体。今后应以入境初期和新婚阶段跨境婚姻家庭为重点干预对象,依托边民通婚备案管理和基层卫生服务网络,加强面向外籍配偶的健康教育和HIV检测服务供给,以提高服务的可及性与公平性。

     

    Abstract: Objective To investigate the current status of HIV-related health service uptake among cross-border couples (Chinese husbands and Burmese wives) in Dehong Dai and Jingpo Autonomous Prefecture, and to provide evidence for optimizing HIV prevention and control strategies in border areas. Methods A cross-sectional survey was conducted from September 2022 to September 2023 among 14 662 cross-border couples (Chinese husbands and Burmese wives) in Dehong Prefecture. Data on sociodemographic characteristics, HIV/AIDS knowledge awareness, and utilization of HIV-related health services within the past year were collected. Multivariable logistic regression was used to analyze factors associated with HIV testing uptake among husbands and wives. Results A total of 29 324 individuals (14 662 couples) were included. The overall awareness rate of HIV/AIDS was 82.09%, with the rate among husbands significantly higher than that among wives (85.17% vs 79.01%, P<0.001). Overall, 94.98% of participants had received at least one HIV-related health service, and the HIV testing rate in the past year was 65.52%. Multivariable analysis showed that, compared with participants married for 0-3 years, those married for 4-10 years and ≥ 11 years were more likely to have received HIV testing. The corresponding ORs were 1.344 (95% CI: 1.187-1.521) and 1.381 (95% CI: 1.211-1.574) among husbands, and 1.308 (95% CI: 1.155-1.481) and 1.344 (95% CI: 1.178-1.533) among wives, respectively. Compared with those holding a marriage certificate, those possessing a border marriage registration certificate were positively associated with HIV testing uptake with ORs of 1.710 (95% CI: 1.575-1.857) among husbands and 1.700 (95% CI: 1.565-1.847) among wives. Compared with participants who were illiterate, those with primary school education showed lower testing rates, with ORs of 0.738 (95% CI: 0.650-0.837) among husbands and 0.827 (95% CI: 0.766-0.891) among wives. Additionally, a history of sexually transmitted diseases (OR=3.66, 95%CI: 1.24-15.59) and participation in the New Rural Cooperative Medical Scheme (OR=1.51, 95%CI: 1.18-1.94) were independently associated with higher testing rates among husbands. Conclusion The overall utilization of HIV-related health services among cross-border couples in Dehong Prefecture is generally satisfactory, but significant gender disparities exist, with foreign wives showing lower rates of knowledge awareness and health service utilization compared with their Chinese husbands. Couples with shorter marriage duration, with primary school education, and those not included in the border marriage registration system are vulnerable groups for HIV testing services. Future interventions should prioritize cross-border marriage families at the early stage of entry and in the newly married period. Based on the border marriage registration management system and primary healthcare networks, health education and HIV testing services targeting foreign spouses should be strengthened to improve service accessibility and equity.
