2023年云南省德宏州手足口病病原检测及基因特征分析

Pathogen detection and genetic characteristics analysis of hand-foot-mouth disease in Dehong Prefecture of Yunnan Province in 2023

  • 摘要:目的】 对2023年云南省德宏州手足口病(HFMD)病例标本进行基因测序定型并对柯萨奇病毒A组4型(CVA4)、6型(CVA6)、10型(CVA10)、16型(CVA16)及肠道病毒71型(EV-A71)的基因特征进行分析,阐明HFMD本土流行的肠道病毒血清型和CVA4、CVA6、CVA10、CVA16和EV-A71的基因型构成,为监测这5种病毒基因型的动态变化提供基线基因型数据。【方法】 对2023年德宏州经实时荧光PCR法鉴定为EV-A71、CVA16和通用型阳性的94份HFMD病例粪便标本先进行VP4/VP2结合区基因测序定型,确定病毒型别,再用肠道病毒对应组别(即肠道病毒A组、B组和C组) VP1区基因引物分两段扩增各病毒VP1区基因全序并测序(A组病毒引物: 486/488和487/489 ,B组病毒引物: 490/492和491/493,C组病毒引物: 494/496和495/497)。按文献下载CVA4、CVA6、CVA10、CVA16和EV-A71病毒VP1基因代表株参考序列,用Mega 6.0软件构建VP1区基因进化树,进行基因特征分析。【结果】 2023年德宏州共采集手足口粪便标本256份,其中实时荧光RT-PCR法核酸阳性94份,阳性率为36.72%(94/256)。94株病毒中,A组病毒83株5个血清型(17株CVA4、3株CVA6、6株CVA10、49株CVA16、8株EV-A71) ; B组病毒9株2个血清型:埃可病毒3型(E3) 2株,柯萨奇病毒B组5型(CVB5) 7株; C组病毒2株1个血清型:脊髓灰质炎病毒3型(Poliovirus 3,PV3) 2株。VP1区基因特征分析表明,17株CVA4均为C2亚型,3株CVA6均为D3a亚型,6株CVA10均为基因型C,49株CVA16中46株为B1a亚型,3株为B1b亚型,8株EV-A71均为C4a亚型。【结论】 2023年德宏州手足口病病原谱主要以A组病毒为主,其中CVA16是检出率最高的病毒,占阳性样本的52.13%(49/94)。基因特征分析表明,CVA4、CVA6、CVA10和EV-A71均为单一基因型(亚型)组成,而CVA16由两个亚型组成(B1a、B1b),B1a是主要的流行亚型。今后继续加强德宏州HFMD病原体尤其是A组病毒的分子流行病学研究具有重要意义。

     

    Abstract: Objective To analyze the genetic characteristics of Coxsackievirus A4 (CVA4), CVA6, CVA10, CVA16 and Enterovirus A71 (EV-A71) in specimens of hand- foot-mouth disease (HFMD) cases in Dehong Prefecture, Yunnan Province in 2023. To clarify the genotype composition of enterovirus serotypes and CVA4, CVA6, CVA10, CVA16 and EV-A71 prevalent in HFMD, and to provide baseline genotype data for monitoring the dynamic changes of these five virus genotypes.Methods 94 stool specimens of HFMD cases identified as EV-A71, CVA16 and universal positive by real-time fluorescent PCR in Dehong Prefecture in 2023 were first sequenced and typed for VP4/VP2 binding region to determine the virus type. The VP1 gene of each virus was amplified and sequenced in two segments by using the VP1 gene primers of the corresponding groups of enteroviruses (i.e., enterovirus A group, B group and C group) (group A virus primers: 486/488 and 487/489, group B virus primers: 490/492 and 491/493, group C virus primers: 494/496 and 495/497). The VP1 gene reference sequences of CVA4, CVA6, CVA10, CVA16 and EV-A71 virus representative strains were downloaded according to the literature. The VP1 gene phylogenetic tree was constructed by Mega 6.0 software, and the genetic characteristics were analyzed.Results In 2023, a total of 256 hand-foot-mouth stool samples were collected in Dehong Prefecture, of which 94 were positive for nucleic acid by real-time fluorescent RT-PCR, with a positive rate of 36.72% (94/256). Among 94 strains of virus, 83 strains of group A virus had 5 serotypes (17 strains of CVA4,3 strains of CVA6,6 strains of CVA10,49 strains of CVA16,8 strains of EV-A71); there were 9 strains of group B virus and 2 serotypes: 2 strains of Echovirus type 3 (E3), 7 strains of Coxsackievirus B type 5 (CVB5); there were 2 strains of group C virus and 1 serotype: 2 strains of poliovirus 3 (PV3). The genetic characteristics of VP1 region showed that 17 strains of CVA4 were C2 subtype, 3 strains of CVA6 were D3 a subtype, 6 strains of CVA10 were genotype C, 46 strains of CVA16 were B1 a subtype, 3 strains were B1 b subtype, and 8 strains of EVA71 were C4 a subtype.Conclusion In 2023, the pathogenic spectrum of HFMD in Dehong Prefecture was mainly group A virus, of which CVA16 was the highest detection rate virus, accounting for 52.13% (49/94) of the positive samples. Genetic characteristics analysis showed that CVA4, CVA6, CVA10 and EV-A71 were composed of a single genotype (subtype), while CVA16 was composed of two subtypes (B1a, B1b), and B1a was the main epidemic subtype. It is of great significance to continue to strengthen the molecular epidemiological study of HFMD pathogens, especially group A viruses in Dehong Prefecture in the future.
