Investigation of a Gymnopilus dilepis poisoning incident in Chongqing

  • 摘要: 目的 对2025年重庆市1起因误食野生蘑菇的中毒事件开展现场流行病学调查,明确致病菌种及毒素类型,为该类事件的处置及临床救治提供参考。方法 采用流行病学调查方法收集病例资料,通过形态学特征初步鉴定蘑菇类型,进一步开展分子鉴定,采用超高效液相色谱—串联质谱法(UPLC-MS/MS)检测蘑菇中的脱磷裸盖菇素及裸盖菇素毒素,同时检测患者血浆中的脱磷裸盖菇素。结果该事件共有3人中毒,临床症状为头晕、乏力、上半身发麻,肝肾功能指标未见异常。结合现场流行病学调查、形态学鉴定及分子鉴定,确定可疑蘑菇为热带紫褐裸伞。实验室检测结果显示,该蘑菇样本中含有裸盖菇素及脱磷裸盖菇素,且在患者血浆中亦检出脱磷裸盖菇素。临床表现与实验室检测结果相符,符合裸盖菇素中毒的诊断特征。结论 热带紫褐裸伞是本次中毒事件的致病蘑菇,其主要致病毒素为裸盖菇素及其代谢物脱磷裸盖菇素,临床表现以神经系统症状为主,肝肾功能无明显损害。建议加强野生蘑菇的识别宣传,预防类似事件发生。

     

    Abstract: Objective To conduct a field epidemiological investigation of a poisoning incident caused by the accidental ingestion of wild mushrooms in Chongqing, and to identify the mushroom species and toxin type, so as to provide references for the management and clinical treatment of such incidents. Methods Epidemiological investigation methods were used to collect case data. The type of mushroom was preliminarily identified based on morphological characteristics. Further molecular identification was conducted. Ultra-high performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) was used to detect the psilocybin and psilocin toxins in the mushroom samples, and psilocin was also detected in the plasma of the patients. Results Three individuals were poisoned in this incident. The clinical manifestations included dizziness, fatigue, and numbness in the upper body, and no abnormalities were observed in liver or kidney function indicators. Based on field epidemiological investigations, morphological identification, and molecular identification, the suspected mushroom was determined to be Gymnopilus dilepis. Laboratory testing detected psilocybin and psilocin in the mushroom samples, and psilocin was also detected in the plasma of the patients. The clinical manifestations were consistent with the laboratory test results, conforming to the diagnostic characteristics of psilocybin poisoning. Conclusion Gymnopilus dilepis is the mushroom responsible for this poisoning incident, with psilocybin and its metabolite psilocin being the primary toxins. The clinical presentation is predominantly characterized by neurological symptoms, with no significant impairment of liver or kidney function. Education on the identification of wild mushrooms should be strengthened to prevent the occurrence of similar incidents.
