社区精神分裂症照顾者负担与患者病耻感的关联研究

Association Between Caregiver Burden and Patient Stigma Among Community-Dwelling Schizophrenia Caregivers

  • 摘要: 目的:探讨精神分裂症家庭照顾者负担与患者病耻感的关系,并分析照顾者的特质正念与感知社会支持在其中的链式中介作用。
    方法:采取方便抽样方法,于2023年8—12月对上海市长宁区和黄浦区357对精神分裂症患者及其主要家庭照顾者进行横断面调查 。采用慢性疾病病耻感量表简版(SSCI-8)、照顾者负担量表(CBI)、正念注意觉知量表(MAAS)和多维感知社会支持量表(MSPSS)评估相关指标 。运用Pearson相关分析、分层回归分析及Bootstrap方法检验链式中介效应 。
    结果:精神分裂症患者病耻感与照顾者负担呈显著正相关(r=0.516,P<0.001),与特质正念(r=-0.412,P<0.001)和感知社会支持(r=-0.317,P<0.001)均呈显著负相关。中介效应分析显示,特质正念、感知社会支持以及二者构成的链式路径在患者病耻感与照顾者负担之间均发挥显著中介作用,其中总中介效应占总效应的44.6%。
    结论:精神分裂症患者病耻感与家庭照顾者负担呈显著正相关,照顾者的特质正念与感知社会支持在其中发挥显著的链式中介作用。提示正念干预及强化家庭与社区社会支持网络可能有助于缓解患者病耻感对照顾者负担的负性影响,从而改善照顾者的心理健康。

     

    Abstract: Objective: To explore the relationship between family caregiver burden in schizophrenia and patients' stigma, and to analyze the chain mediating roles of caregivers' trait mindfulness and perceived social support in this relationship.Methods: A cross-sectional survey was conducted using a convenience sampling method among 357 pairs of patients with schizophrenia and their primary family caregivers in Changning and Huangpu Districts of Shanghai from August to December 2023. Relevant variables were assessed using the Stigma Scale for Chronic Illness 8-item version (SSCI-8), the Caregiver Burden Inventory (CBI), the Mindful Attention Awareness Scale (MAAS), and the Multidimensional Scale of Perceived Social Support (MSPSS). Pearson correlation analysis, hierarchical regression analysis, and Bootstrap method were employed to test the chain mediating effect.Results: Patient stigma was significantly positively correlated with caregiver burden (r = 0.516, P < 0.001), and significantly negatively correlated with both trait mindfulness (r = -0.412, P < 0.001) and perceived social support (r = -0.317, P < 0.001). Mediating effect analysis revealed that trait mindfulness, perceived social support, and their combined chain pathway all exerted significant mediating effects between patient stigma and caregiver burden, with the total mediating effect accounting for 44.6% of the total effect.Conclusions: Patient stigma in schizophrenia was significantly positively associated with family caregiver burden, and caregivers’ trait mindfulness and perceived social support played significant chain mediating roles in this relationship. These findings suggest that mindfulness-based interventions and the enhancement of family and community social support may help mitigate the negative impact of patient stigma on caregiver burden, thereby improving caregivers’ psychological well-being.
