Associations Between Loneliness, Depression, and Lifestyle Factors Among Older Adults in Retirement Homes of Huangpu District, Shanghai

  • 摘要:目的】探讨上海市黄浦区养老院老年人孤独感与抑郁的现状及其与生活方式因素之间的关系,为养老院老年人心理健康促进提供依据。【方法】于2023年9月至2024年4月,采用方便抽样方法,对上海市黄浦区8家养老院382名老年人进行问卷调查。调查内容包括人口学特征、生活方式、孤独感和抑郁状况。孤独感采用De Jong-Gierveld 11项孤独量表测量,抑郁状况采用医院焦虑抑郁量表抑郁分量表测量。采用Spearman相关分析、t检验、单因素方差分析及多元线性回归分析养老院老年人孤独感和抑郁的相关因素。【结果】养老院老年人孤独感与抑郁得分呈显著正相关(r=0.379,P<0.001)。多元线性回归分析显示,家属探视频率(β=-0.112,P<0.05)和晒太阳频率(β=-0.320,P<0.001)与孤独感得分显著负相关;每周身体活动频率(β=-0.281,P<0.001)、晒太阳频率(β=-0.192,P<0.001)及孤独感水平(β=0.253,P<0.001)与抑郁得分显著相关。孤独感模型和抑郁模型分别解释13.2%和31.5%的变异量。【结论】养老院老年人孤独感和抑郁问题较为突出,且与多种生活方式因素密切相关。通过促进规律身体活动、增加家属探视和鼓励日常晒太阳等措施,可能有助于改善养老院老年人的心理健康状况。

     

    Abstract: ObjectiveTo examine the status of loneliness and depression among older adults in nursing homes in Huangpu District, Shanghai, and their associations with lifestyle factors, and to provide evidence for promoting mental health among institutionalized older adults. Methods From September 2023 to April 2024, a questionnaire survey was conducted among 382 older adults from eight nursing homes in Huangpu District, Shanghai, using convenience sampling. The survey collected information on demographic characteristics, lifestyle factors, loneliness, and depression. Loneliness was assessed using the 11-item De Jong-Gierveld Loneliness Scale, and depression was measured using the depression subscale of the Hospital Anxiety and Depression Scale. Spearman correlation analysis, independent-samples t tests, one-way analysis of variance, and multiple linear regression analyses were performed to examine factors associated with loneliness and depression. Results Loneliness was significantly and positively correlated with depression among older adults in nursing homes (r = 0.379, P < 0.001). Multiple linear regression analyses showed that family visit frequency (β = -0.112, P < 0.05) and sun exposure frequency (β = -0.320, P < 0.001) were significantly and negatively associated with loneliness. Weekly physical activity frequency (β = -0.281, P < 0.001), sun exposure frequency (β = -0.192, P < 0.001), and loneliness level (β = 0.253, P < 0.001) were significantly associated with depression. The loneliness and depression models explained 13.2% and 31.5% of the variance, respectively. Conclusion Loneliness and depression are prevalent among older adults in nursing homes and are closely associated with multiple lifestyle factors. Promoting regular physical activity, increasing family visits, and encouraging daily sun exposure may help improve the mental health of older adults in nursing homes.
