2023年上海市浦东新区3~5岁儿童龋病现况及影响因素分析

Dental caries and its influencing factors among 3–5-year-old children in Pudong New Area of Shanghai in 2023

  • 摘要:目的】分析上海市浦东新区3~5岁学龄前儿童患龋率及影响因素,为制定浦东新区学龄前儿童的口腔龋病防治策略提供依据。【方法】采用横断面设计,通过多阶段整群随机抽样方法,于2023年10—12月在上海市浦东新区9所幼儿园中抽取1 195名3~5岁儿童进行现场口腔检查和问卷调查,并运用多因素logistic回归模型分析影响低龄儿童龋(ECC)的相关因素。【结果】共纳入有效样本1 083例,儿童患龋率为39.06%。多因素logistic回归模型分析结果显示,与年龄为3岁、使用含氟牙膏、碳酸饮料频率<1次·周-1、晚上刷牙后进食频率<1次·周-1的儿童相比,年龄为5岁(OR=2.199,95% CI:1.433~3.374,P<0.001)、4岁(OR=1.800,95% CI:1.346~2.407,P<0.001)、未使用含氟牙膏(OR=2.334,95% CI:1.749~3.116,P<0.001)、碳酸饮料频率≥1次·周-1(OR=1.591,95% CI:1.157~2.188,P=0.004 )、晚上刷牙后进食频率≥1次·周-1(OR=1.571,95% CI:1.129~2.184,P=0.007 )的儿童患龋风险更高;与完全母乳喂养的儿童相比,喂养方式为母乳和人工喂养各半的儿童患龋风险更低(OR=0.554,95% CI:0.368~0.836,P=0.005)。【结论】2023年浦东新区3~5岁学龄前儿童患龋率与国际先进水平相比仍有一定差距,年龄、6月龄内喂养方式、是否使用含氟牙膏、碳酸饮料频率和晚上刷牙后进食频率均与儿童患龋相关。建议依托幼儿园开展针对性健康教育,强化监护人的行为指导,切实保障学龄前儿童的口腔健康。

     

    Abstract: Objective To analyze the prevalence of dental caries and its influencing factors among preschool children aged 3-5 years old in Pudong New Area of Shanghai, and to provide evidence for developing caries prevention strategies for this population. Methods A cross-sectional design was adopted. Using multistage cluster random sampling, 1,195 children aged 3-5 years old were selected from 9 kindergartens in Pudong New Area of Shanghai from October to December 2023 for on-site oral examinations and questionnaire surveys. A multivariate logistic regression model was used to analyze factors associated with early childhood caries (ECC). Results A total of 1,083 participants were included, with a caries prevalence of 39.06%. Multivariate logistic regression analyses showed that, compared with children who were aged 3 years old, used fluoride toothpaste, consumed carbonated beverages less than once per week, and ate after nighttime tooth brushing less than once per week, those who were aged 5 years old (OR = 2.199, 95% CI: 1.433-3.374, P < 0.001), aged 4 years old (OR = 1.800, 95% CI: 1.346-2.407, P < 0.001), did not use fluoride toothpaste (OR = 2.334, 95% CI: 1.749-3.116, P < 0.001), consumed carbonated beverages ≥1 time per week (OR = 1.591, 95% CI: 1.157-2.188, P = 0.004), and ate after nighttime tooth brushing ≥1 time per week (OR = 1.571, 95% CI: 1.129-2.184, P = 0.007) were at a higher risk of dental caries. Compared with children who were exclusively breastfed, those with mixed feeding (half breastfeeding and half formula feeding) had a lower risk of caries (OR = 0.554, 95% CI: 0.368-0.836, P = 0.005). Conclusion In 2023, the caries prevalence among preschool children aged 3-5 years old in Pudong New Area still lagged behind advanced international levels. Age, feeding pattern within the first 6 months of life, use of fluoride toothpaste, frequency of carbonated beverage consumption, and eating after nighttime tooth brushing were all associated with childhood caries. It is recommended to implement targeted health education in kindergartens, strengthen behavioral guidance for caregivers, and effectively safeguard the oral health of preschool children.
