慢性阻塞性肺疾病急性加重与环境因素的相关性研究

Correlation of acute exacerbation of chronic obstructive pulmonary disease with environmental factors

  • 摘要: 目的 探讨不同程度肺功能损害的慢性阻塞性肺疾病(COPD)急性加重与大气质量、气温等环境因素的关系。方法 收集2004年1月1日-2009年7月31日,上海一所中心医院收住入院的慢性阻塞性肺疾病急性加重的病例,按入院后肺功能检查结果分成轻度、中度、重度3组。分析每组每日入院人数与入院当天(d0)、前1天(d-1)、前3天(d-3)、前6天(d-6)的大气污染物浓度、气温之间的相关性。结果 肺功能重度损害COPD组每日入院人数与大气质量因素均呈现一定的正相关。与最高、最低气温呈一定的负相关。肺功能中度损害COPD组每日入院人数与入院前3天PM10的API值呈正相关,与最低气温呈负相关。肺功能轻度损害COPD组每日入院人数与入院当天的SO2的API值呈正相关,与最低气温呈负相关。结论 慢性阻塞性肺疾病急性加重的发生与大气质量、气温存在一定的相关性,尤其是对于肺功能重度损害的COPD病人。提示大气污染、气温降低是慢性阻塞性肺病急性加重的重要诱因。

     

    Abstract: Objective To explore the relationship of acute exacerbation of chronic obstructive pulmonary disease (AECOPD) with environmental factors such as air quality and temperature. Methods From January 2004 to July 2009, hospitalized patients of AECOPD were collected from central hospitals.According to the result of pulmonary function test, the cases were divided into three groups:mild, moderate and severe ones.The analyses were made between the daily number of admitted patients in each group and the density of atmospheric pollutant and temperature which were measured on admission, and one day, three days, six days before admission respectively. Results On the whole, the daily number of admitted patients in severe COPD group showed positive correlation with API value of atmospheric pollutant irrespective of timing, and the same negative correlation with the maximum temperature or the lowest temperature.Moderate COPD group had positive correlation with the density of PM10 on d3, and showed the same negative correlation with the lowest temperature.Mild COPD group had positive correlation with the density API value of SO2 on d0, and showed negative correlation with the lowest temperature. Conclusion Our study found that to some extent, the onset of AECOPD had correlation with air quality and temperature especially for patients whose pulmonary functions were severely impaired.This indicated that air quality and temperature were important factors for AECOPD.
