Progress and prospects of the pilot program of family doctor contract and service in Shanghai

  • 摘要:
    目的回顾上海市家庭医生签约服务历程,分析工作进展及存在的主要问题,提出建议与设想,为试点工作在全市推广与深化提供参考。
    方法依据上海市社区卫生服务综合改革云管理APP,采集并分析上海市各区社区卫生服务中心试点工作相关数据,并通过专家咨询法,对知情人进行访谈。
    结果上海市全部239家社区卫生服务中心均已启动了“1+1+1”签约服务试点,签约居民超过335万人,其中60岁及以上老年人签约率达到65.80%;为签约居民开具延伸处方超过91万张;签约居民门诊在“1+1+1”签约医疗机构组合内就诊为74.69%,在社区卫生服务中心就诊接近70%,在签约社区就诊达53.84%。
    结论签约试点进展良好,但仍然存在各社区试点进度不一、就诊频次过高、家庭医生签约服务积极性受限及供给模式单一等问题,需要从试点推广、落实服务举措、夯实运行机制、借助信息技术等方面进一步完善。

     

    Abstract:
    ObjectiveBy reviewing the service history of Shanghai family doctor, by analyzing the current progress and existing problems, to put forward the suggestions and ideas for the next step.
    MethodsAccording to Shanghai Community Health Services Comprehensive Reform Management APP, the data on all community health service center pilot progress of the relevant collected, aggregated and analysed, and the focus group was interviewed through expert advising method.
    ResultsAll community health service centers in Shanghai have launched the "1+1+1" contract and service pilot program, with the number of residents with "1+1+1" signing more than 3.35 million, of which 65.80% of the elderly people over the age of 60 have signed up; contracted residents clinics visited 74.69% in the "1+1+1" contracted medical institutions, nearly 70% visited community health service centers, and reached 53.84% in contracted community clinics.
    ConclusionThe pilot project is progressing well but there are still some problems such as the pilot progress, the frequency of visits, the enthusiasm of contracting services of family doctors and the single mode of supply of subscription services.The measures taken need to be based on the pilot promotion, the implementation of service initiatives, the consolidation of operational mechanisms and the use of information technology need to be further improved.
