上海地区孕早中期孕妇睡眠质量现况及相关的社会结构性因素

Sleep quality among pregnant women of first or second trimester in Shanghai:its current situation and social structural factors

    目的了解上海地区孕早、中期孕妇的睡眠质量及其社会结构性因素的影响,为有针对性地提出妇幼保健服务改善策略提供参考。
    方法对2016年4月—2017年6月在上海市2家接产医院首次建卡的孕早、中期孕妇,采用匹兹堡睡眠指数量表评价近1个月的睡眠质量,并通过问卷调查了解其学历、工作状况、家庭经济水平、户籍、在上海居住时间等社会结构性因素,以及可能影响睡眠质量的多种混杂因素。
    结果被调查孕妇总体睡眠障碍检出率为58.3%,主观睡眠质量差和白天功能紊乱是主要问题。与在上海居住小于5年的孕妇相比,在上海居住26年及以上的孕妇睡眠障碍可能性增加36.1%;与家庭年收入超过20万元的孕妇相比,家庭年收入为10万~20万元者睡眠障碍可能性增加29.5%;与有固定工作的孕妇相比,没有工作的孕妇睡眠障碍可能性增加18.1%。
    结论在现有的社会经济坏境下,上海地区孕早、中期孕妇的睡眠质量总体情况不容乐观,应重点关注那些没有工作、家庭年收入中等、在上海居住时间较长孕妇的睡眠健康。

     

    Abstract:
    ObjectiveTo investigate sleep qualities and their social determinants among pregnant women of the first or second trimester in Shanghai and to provide evidence for improvement on strategy for maternal, child and adolescent health in Shanghai.
    MethodsSleep quality of pregnant women in their first or second trimester recruited from two hospitals from April 2016 to June 2017 in Shanghai, was evaluated by Pittsburgh Sleep Quality Index.Self-administered questionnaires were also used to obtain social structural factors such as pregnant women's educational levels, working status, family economy status, household register, and dwelling time in shanghai, with potential confounding factors of sleep included.
    ResultsThe proportion of participated pregnant women who were found to have sleep disturbance was 58.3%.Main sleep problems were poor subjective sleep quality and daytime function disturbance.Compared with those who lived in Shanghai less than 5 years, subjects who lived more than 26 years had likelihood of sleep disturbance significantly increased by 36.1%.In comparison with those having annual household income over 200, 000 yuan, the likelihood of sleep disturbance for subjects with annual household income of 100, 000 to 200, 000 yuan was statistically increased by 29.5%.In comparison with pregnant women who had stable work, subjects who did not have stable work would have the likelihood of sleep disturbance increased by 18.1%.
    ConclusionIn the context of current social and economic development, the sleep quality of pregnant women in Shanghai is not optimistic.Sleep health of pregnant women should be promoted, especially for those who have no stable work, moderate household income, long-term residence in Shanghai.
