2011—2016年浦东地区23 405例孕妇TORCH感染情况及相关因素分析

TORCH infection state and related factors in 23 405 pregnant women in Pudong Area from 2011 to 2016

    目的了解2011至2016年上海浦东地区育龄妇女弓形虫、风疹病毒、巨细胞病毒和单纯疱疹病毒(简称“TORCH”)感染情况及相关因素。
    方法采用化学发光法对2011至2016年浦东妇幼保健院健康检查的23 405例妊娠妇女进行TORCH检测,统计TORCH感染与育龄妇女年龄、户籍及孕期宠物接触史的相关性。
    结果23 405例样本中, 巨细胞病毒(CMV)IgG、风疹病毒(RV)IgG、RV-IgM、弓形虫(TOX)IgG、CMV-IgM、TOX-IgM检测阳性率分别为87.54%, 66.77%、1.32%、0.99%、0.47%和0.36%。历年弓形虫感染阳性率呈递增趋势(P<0.05)。其中TOX-IgG阳性率年均增长11.36%,TOX-IgM阳性率年均增长19.67%。RV-IgG,TOX-IgG,RV-IgM和TOX-IgM的阳性率未见上升趋势(P均>0.05)。TORCH感染与宠物接触史有统计学关联(P<0.05);弓形虫、风疹病毒感染与户籍来源、孕妇年龄相关(P<0.05)。
    结论近年来育龄妇女TORCH感染呈逐年递增趋势,其主要原因为弓形虫阳性率的上升;饲养宠物、农村户籍均为TORCH感染的高危因素。强化育龄人群的优生优育意识,加强对高龄妊娠妇女、饲养宠物者及农村户籍孕产妇的TORCH感染检测。

     

    Abstract:
    ObjectiveTo investigate the TORCH(Toxoplasma gondii, Rubella virus, Cytomegalo-virus, and Herpes simplex virus) infections status and related factors among pregnant women in Pudong Area, during 2011-2016.
    MethodsChemiluminescence method was used to detect TORCH infections in 23 405 pregnant women who visited Health Hospital for Women and Children of Shanghai Pudong Area from 2011 to 2016.The correlations between TORCH infections and the household registration, pet contact history and pregnant age were analyzed accordingly.
    ResultsIn 23 405 samples, the positivity of Cytomegalo virus Immunoglobulin G (IgG), Rubella virus IgG, Rubella virus (Immunoglobulin M) IgM, Toxoplasma gondii IgG, Cytomegalo virus IgM, Toxoplasma gondii IgM was 87.54%, 66.77%, 1.32%, 0.99%, 0.47% and 0.36%, respectively.Upward trend in Toxoplasma gondii positivity was observed during the study period (P < 0.05).The average annual positivity of TOX-IgG and TOX-IgM increased 11.36% and 19.67%, respectively.No statistical difference was found in the positivity of Rubella virus and Cytomegalo virus in the past years.There was statistical correlation between TORCH infections and the pet contact history (P < 0.05).The age of pregnancy, household registry and pet contact history during pregnancy were significantly associated with infections of Toxoplasma gondii and Rubella virus (P < 0.05 for all).
    ConclusionTORCH infection rate was increasing in pregnant women in the past years, which were mainly due to the increased Toxoplasma gondii positivity.Feeding pets, rural household registration are the risk factors for TORCH infection.Strengthening the examination of TORCH infections among pregnant women and applying effective medical intervention are of great importance for ensuring the health of pregnant women and fetuses.
