2017-2018年奉贤区公共卫生苗子事件的流行特征分析

Epidemiological characteristics of public health seedling events in Fengxian District from 2017 to 2018

    目的分析奉贤区2017-2018年公共卫生苗子事件的流行病学特征,为今后重点公共卫生苗子事件预防控制工作提供依据。
    方法收集2017-2018年奉贤区苗子事件数据资料,应用描述性流行病学方法进行分析。
    结果2017年1月1日-2018年12月31日共新增报告公共卫生苗子事件141起,其中2起为突发公共卫生事件,以麻疹、手足口病、水痘、诺如病毒感染性腹泻、流感和肺结核这6种传染病为主,大多数分布于奉贤人口聚集性的中部和西部地区,3-6月份和9-12月份是苗子事件发生高峰,托幼机构与小学的苗子事件波及人数占比达到了65.97%,2018年比2017年苗子事件数增加41起,其中手足口病占23起,发病到接报、接报到结案和发病到结案的间隔时间以中位数(P25~P75)表示分别为3(2~8)d,11(8~14)d,15(11~22.5)d。
    结论应重点关注奉贤区人口聚集的中部和西部地区, 重点病种为麻疹、手足口病及水痘。疾控部门在高发季节前对重点集体单位(特别是托幼机构和小学)做好针对性培训和督导指导工作,并根据历年发病情况进行适当预测和预警并告知相关人员,在苗子事件发生后,根据不同传染病的潜伏期、病程等做好后续处置管理工作。

     

    Abstract:
    ObjectiveTo analyze the epidemiological characteristics of public health seedling events in Fengxian District from 2017 to 2018, and to provide evidence for the prevention and control of key public health seedling events in future.
    MethodsData on seedling incidents in Fengxian District from 2017 to 2018 were collected and analyzed by descriptive epidemiological method.
    ResultsFrom January 1, 2017 to December 31, 2018, a total of 141 new public health seedling events were reported, of which 2 were public health emergencies. The main diseases were measles, hand, foot and mouth disease, chickenpox, norovirus infection, influenza and tuberculosis. Most of them were in the central and western areas where Fengxian population was clustered. The public health seedling events occurred in the peak of March-June and September-December. The number of incidents involving nursery institutions and primary schools accounted for 65.97%. In 2018, the number of incidents involving nursery schools increased by 41 cases. Of them, hand, foot and mouth disease accounted for 23. The median interval between the onset and event report, event report and closure of events(P25-P75)was 3(2-8)days, 11(8-14)days and 15(11-22.5)days, respectively.
    ConclusionWe should pay more attention to the central and western areas where population gathers in Fengxian District. We should pay more attention to measles, hand, foot and mouth disease, chickenpox. CDC departments should train and supervise key collective units(especially kindergartens and elementary schools)before the key seasons, and make appropriate prediction and early warning according to the incidence over the years, and inform relevant personnel that after the occurrence of the seedling events. According to the incubation period and duration of different infectious diseases, follow-up management work should be done well.
