Evaluation of the measles surveillance system performance in Jiading District, Shanghai from 2020 to 2022

    目的 通过分析2020—2022年上海市嘉定区麻疹监测系统(MSS)监测结果,为嘉定区麻疹消除工作提供参考。
    方法 结合实验室结果和流行病学调查资料对上海市嘉定区2020—2022年麻疹监测系统结果和麻疹确诊病例进行描述性分析。
    结果 2020—2022年上海市嘉定区麻疹监测系统共报告疑似病例120例,其中实验室确诊麻疹病例12例,实验室确诊风疹病例9例,排除麻疹、风疹病例 99例,2020、2021及2022年排除麻疹、风疹病例报告率分别为1.44/10万、2.01/10万和1.99/10万。48 h完整调查率、血清标本和病原学采集率、送达及时率和报告及时率均为100%。2020—2022年12例确诊病例中7例常规免疫对象均按要求完成麻疹类疫苗接种剂次, 5例成人有2例有明确麻疹类疫苗相关接种史。确诊病例中发热伴出疹病例6例,仅出疹病例5例,仅发热病例1例。
    结论 上海市嘉定区麻疹监测结果显示监测系统运转总体良好,但敏感性仍须进一步提高,特别对非典型症状病例的发现和报告。仍须继续保持适龄儿童麻疹类疫苗高覆盖率,进一步开展疫苗补充免疫活动,提高全人群免疫水平。

     

    Abstract:
    Objective To evaluate the measles surveillance system (MSS) in Jiading District, Shanghai from 2020 to 2022, and to provide evidence for the elimination of measles.
    Methods Descriptive methods were used to analyze the MSS data and confirmed measles cases from 2020 to 2022 and to evaluate MSS performance indicators.
    Results A total of 120 suspected cases were reported through the MSS from 2020 to 2022, of which 12 were classified as measles, 9 as rubella, and 99 as non-measles /rubella. The incidence of reported non-measles /rubella was 1.44 per 100 000 population in 2020, 2.01 per 100 000 population in 2021, and 1.99 per 100 000 population in 2022. The rates of complete investigation within 48 hours, blood samples and etiology collection, timely delivery, and timely reporting were all 100%. Among the 12 confirmed measles cases from 2020 to 2022, seven routine immunization subjects completed the required doses of measles vaccines, while two out five adult cases had a history of measles vaccine-related immunization. The confirmed cases comprised six with fever accompanied by rash, five with rash alone, and one with fever alone.
    Conclusion The MSS results in Jiading District, Shanghai are overall satisfactory. However, there is a need to improve sensitivity, especially in detecting and reporting cases with atypical symptoms. It is imperative to maintain high vaccination coverage for age-appropriate children, promote supplementary immunization activities, and elevate the overall immunity of the entire population.
