舟山市某院青少年幽门螺杆菌感染及耐药性调查

Helicobacter Pyloris infection and its antibiotic resistance in adolescents at a hospital of Zhoushan City

  • 摘要: 目的了解舟山群岛地区青少年幽门螺杆菌(Hp)感染情况及Hp对常用抗生素的耐药性。方法对2008年3月-2011年5月因上消化道疾病在舟山市妇幼保健院住院和门诊治疗的216例青少年进行胃镜检查,并取胃黏膜标本进行Hp培养分离,采用Kirby-Baner药敏纸片对培养分离到的菌株进行抗生素敏感性试验。结果216例中成功分离出Hp菌株71例,阳性率为32.87%,其中6~10岁组阳性率为17.86%(15/84),10~14岁组阳性率为30.95%(13/42),14~18岁组阳性率为48.78%(43/90),三组间差异有统计学意义。克拉霉素耐药率为8.45%(6/71)、阿莫西林耐药率为22.54%(16/71)、甲硝唑耐药率为94.37%(67/71)、呋喃唑酮耐药率为16.90%(12/71),而庆大霉素和左氧氟沙星均未检出耐药菌株。结论本地区青少年胃黏膜Hp培养阳性率随年龄增长而上升,青少年Hp对甲硝唑和阿莫西林耐药率较高,对克拉霉素和呋喃唑酮耐药率较低,而对庆大霉素和左氧氟沙星无耐药菌株。

     

    Abstract: Objective To investigate Helicobacter Pyloris (Hp) infection in adolescents and prevalence of Hp resistance to commonly used antibiotics. Methods A series of 216 cases in our hospital were enrolled between March 1, 2008 and May 31, 2011. The research group did gastroscopy and grastric mucosal samples were taken for culture and isolation of Hp strains. And then the resistance rate to the antibiotics was tested by Kirby-Baner method. Results A total of 71 Hp strains were cultured and isolated from 216 adolescent patients and the positiveness rate was found to be 32.87%. In the group aged 6 years to 10, the positiveness rate was 17.86%(15/84),in the group aged 10 years to 14, 30.95%(13/42) and in the group aged 14 years to 18, 48.78%(43/90). There was markedly statistical difference in Hp infection rate between the three groups. Of the 71 Hp strains isolated from adolescent, the antibiotics resistance rate to Clarithromycin, Amoxicillin, Metronidazole, Fruranzolidone, Gentamicin and Levofloxacin was respectively 8.45%(6/71), 22.54%(16/71), 94.37%(67/71), 16.90%(12/71), 0% (0/71) and 0%(0/71). Conclusion Hp culture positiveness rate is increasing with the age rising, the prevalence of Hp resistance rate in adolescents to Metronidazole and Amoxicillin are relatively higher, as compared to the resistance rate to Clarithromycin and Fruranzolidone. There is no Hp strain found to be resistant to Gentamicin and Levofloxacin.
