中文版童年不良经历量表在男男性行为群体中的信效度分析

Reliability and validation of the Adverse Childhood Experiences-International Questionnaire among men who have sex with men in China

    目的 男男性行为者(MSM)中童年不良经历(ACEs)的重要性不容忽视,本研究旨在分析中文版童年不良经历量表(ACE-IQ)在MSM中的信度和效度,以评估其在该群体中的适用性。
    方法 通过滚雪球抽样法在中国3个城市(上海、沈阳、昆明)开展横断面调查,最终纳入1 130名MSM进行分析。采用探索性因子分析(EFA)和验证性因子分析(CFA)评估量表的结构效度。内部一致性信度通过Cronbach’s α系数评估,分半信度采用Spearman⁃Brown系数评估。
    结果 根据EFA结果删除ACE10“父母去世”这一条目后,23个条目的ACE⁃IQ量表呈现六因子结构,包括情感或身体虐待的受害者/目击者、性虐待、身体忽视、家庭功能障碍、情感忽视和同伴暴力。CFA结果显示,六因子模型的拟合优度较好,支持EFA结果。该量表的Cronbach’s α系数为0.852,Spearman⁃Brown系数为0.899,表明在研究人群中具有较高的内部一致性和分半信度。
    结论 中文版ACE⁃IQ量表在MSM中具有良好的信度和效度,适合作为评估该群体童年不良经历的有效工具,并为进一步的心理健康干预提供了科学依据。

     

    Abstract:
    Objective The significance of adverse childhood experiences (ACEs) among men who have sex with men (MSM) should not be overlooked. This study aims to assess the reliability and validity of the Chinese version of the Adverse Childhood Experiences-International Questionnaire (ACE-IQ) among the MSM population in China to evaluate its applicability in this group.
    Methods A cross-sectional survey was conducted in three Chinese cities(Shanghai, Shenyang and Kunming) using snowball sampling, with a total of 1 130 MSM participants included. Exploratory factor analysis (EFA) and confirmatory factor analysis (CFA) were used to assess the structural validity of the scale. Internal consistency reliability was evaluated using Cronbach’s α coefficient, and split-half reliability was assessed with the Spearman-Brown coefficient.
    Results According to the EFA results, after removing item ACE10 (“parental death”), the 23-item ACE-IQ demonstrated a six-factor structure, including victimization or witnessing emotional/physical abuse, sexual abuse, physical neglect, family dysfunction, emotional neglect, and peer violence. CFA results indicated a good model fit for the six-factor model, supporting the EFA findings. The scale demonstrated good reliability, with a Cronbach's α coefficient of 0.852 and a Spearman-Brown coefficient of 0.899, indicating high internal consistency and split-half reliability within the studied population.
    Conclusion The Chinese version of the ACE-IQ demonstrates satisfactory reliability and validity among MSM population, supporting its suitability for assessing ACEs in this group. The findings provide an empirical basis for subsequent mental health interventions.
