弱阳离子磁珠联合MALDI-TOF-MS在2型糖尿病大血管并发症检测中的应用探讨

Combination of weak cation exchange (WCX) beads and MALDI-TOF-MS in the diagnosis of type 2 diabetes with macrovascular complication

  • 摘要: 目的 弱阳离子(WCX)磁珠联合应用基质辅助激光解析电离化飞行时间质谱(MALDI-TOF-MS)技术分析2型糖尿病大血管并发症患者血清蛋白组,筛选差异蛋白峰并建立决策树模型,初步探讨所建立的决策树模型在2型糖尿病大血管并发症早期鉴别诊断中的临床意义。方法 55例2型糖尿病大血管并发症患者,84例2型糖尿病无大血管并发症患者,采用WCX联合MALDI-TOF-MS技术获得其蛋白质谱,分析差异并建立决策树模型,扩大样本量,通过盲法验证诊断模型的可靠性。结果 建立了由质荷比为16 062.20、31 883.60和7 986.76差异蛋白峰组成的2型糖尿病大血管并发症决策树诊断模型,扩大样本量盲筛敏感性为96.3%,特异性分别为100.0%。结论 应用WCX磁珠联合MALDI-TOF-MS技术可筛选出2型糖尿病大血管并发症患者的差异蛋白峰,建立的决策树模型具有的一定的临床应用前景。

     

    Abstract: Objective To analysis serum protein group in patients with type 2 diabetes with macrovascular complication by application of combination of weak cation exchange (WCX) beads and Matrix-Assisted Laser Desorption/Ionization Time of Flight Mass Spectrometry (MALDI-TOF-MS),to screen the differential protein peaks and establish the decision tree model; and to explore the clinical significance of the decision tree model in early diagnosis of type 2 diabetes with macrovascular complication.Methods This study included 55 patients with type 2 diabetes with macrovascular complication, 84 type 2 diabetic patients without macrovascular complication. Weak cation exchange beads (WCX) combined with MALDI-TOF-MS technique was employed to analyze the serum proteomics. The differential protein peaks were selected to build the decision tree model, and the reliability of the model was verified by blind test after the sample size was expanded.Results Several protein peaks were identified to be strongly correlated with incidence of macrovascular complication in type 2 diabetic patients. In those proteins, three diagnostic markers, i.e.16 062.20 (m/z),31 883.60 (m/z),7 986.76 (m/z) were selected to differentiate those with macrovascular complication or not. Further blind test showed that the sensitivity and specificity of the combined markers were 96.3% and 100%, respectively.Conclusion With the technique of WCX beads combined with MALDI-TOF-MS,several specific protein peaks correlated with the incidence of macrovascular complications in type 2 diabetic patients is successfully identified. These protein peaks might serve as biomarkers for the prognosis of macrovascular complications.
